Job Summary:
Facilitates successful community integration of young people with disabilities.
Education & Experience:
- High School Diploma, high school equivalency, or -
- State Education Commencement Credential (e.g., SAC or CDOS) with related human service experience
- Experience working with families, children/youth.
Other Requirements & Skills:
- Understanding of Trauma Informed Care – ability to operate within an empathic, person-centered framework.
- Recognition of and respect for cultural diversity within the organization and community.
- Completion of Mandated Reporter Training prior to service provision
- Completion of the following trainings within 6 months of hire: Personal Safety, Safety in the Community, Strength Based Approaches, Suicide Prevention, Domestic Violence Signs and Basic Interventions; Trauma Informed Care
- Must have valid driver’s license, insurability, and reliable transportation.
- Minimum personal auto coverage of 100,000 bodily per person/300,000 per accident/ 100,000 property
- Must pass Justice Center, State Central Registry and Criminal background checks.
Primary Responsibilities:
- Program participant enrollment; development and maintenance of Person-Centered service plans with program participants
- Active engagement with program participants/families in the setting of their choice.
- Facilitation of self-advocacy training for program participants to empower them to advocate on behalf of their youth/families; special emphasis will be placed upon community transitions.
- Education for youth and caregivers on success transitions to the community.
- Attendance monthly provider meetings.
- Collaboration and service coordination with community-based providers with an emphasis on meeting participant needs.
- Maintenance records, as required by funder and as needed for coordination of services, billing, and reporting.
