Job Title: CNA, PCA, HHA Caregiver

Schedule: Availability to work on: Tuesday, Thursday, and Fridays from 8:30am to 12:30pm
Job Description: Collaborative Home Care is seeking a compassionate and reliable Caregiver to join our team. As a caregiver, you will play a vital role in enhancing our client's quality of life and providing essential companionship. • Elderly man with dementia
• Assistance with mobility assistance, transfers, showering, dressing, toileting
• Assist with exercises, meal prep, light housekeeping
• No pets/smoking
• Mileage reimbursement for transportation services provided

• Eligible to work in the United States for any employer
• Pass a background check and drug testing per company standards

Responsibilities and Duties: 

• Our Caregivers are responsible for providing hands-on, non-medical, quality care to clients of Collaborative Home Care.
• Caregivers provide care based on the plan of care for their clients, which includes activities of daily living such as personal care (e.g. bathing, dressing, medication reminders)
• Companion care (e.g. conversation, hobbies and activities, shopping).
• Caregivers must be able to work with minimal supervision, accepting personal responsibility for the maintenance of a professional relationship with the client in accordance with Collaborative Home Care policies and standards.
