Job Summary: Responsible for meeting individual needs of each resident as represented on the individual resident care plans. Carries out specific duties and responsibilities related to contributing and improving overall quality of life for each resident through the performance of Essential Job Functions.
Essential 1. Performs routine resident care in accordance with resident care plans and
Functions: assists residents with all activities of daily living (ADLs).
2. Ensures that adequate nutritional and fluid intake is provided to all residents.
3. Prepares meals for the residents as dictated by the weekly dietary menu provided by the registered dietician.
4. Assists with resident activities as scheduled and promotes mental and social stimulation through activities and social interactions with residents.
5. Completes documentation guidelines to reflect care provided on the resident health record.
6. Monitors and reports observations of any indication of changes in resident’s condition during the performance of routine resident care.
Reports promptly, any significant change of a resident's condition or any other concerns, to the Executive Director.
7. Follows all infection control guidelines during the performance of routine resident care.
8. Coordinates and prioritizes resident care activities based on current resident care needs.
9. Completes resident room cleaning, general unit cleaning, laundry and facility cleaning as assigned by shift responsibility schedule.
10. Participates in all training, education and in-services as required.
11. Complies with facility safety policies and practices to promote a safe, accident free work environment.
12. Accurate completion, documentation and follow-up on all resident incidents as mandated by regulatory guidelines.
13. Ensures that resident rights are well-respected and maintained.
14. Maintains confidentiality of all facility operations.
15. Maintains confidentiality of all pertinent resident records and information.
16. Responsible for reporting violations of facility operations and company policies and procedures to the Executive Director.
17. Demonstrates customer service standards for daily interaction with residents, visitors and family members.
Additional Duties
1. Answers the telephone in the absence of the administrative staff and
directs the calls to the appropriate person.
2. Assists with building security while on duty.
3. Assists with transporting and accompanying the resident to physician and any other professional appointments.
4. Participates in the resident care conferences with family members and administrative staff.
5. Participates in staff meetings as directed by the Executive Director.
6. Assists in the distribution of medications as required or directed by licensed personnel.
7. Records, in the communication log, any resident unit or facility issues for information and/or resolution.
Education/ Skill/Experience
1. Must be able to read, write and speak the English language.
2. Must be a high school graduate or have equivalent certification.
3. Must be eighteen years of age or over.

4. Must possess the ability to apply basic principals or resident care based on the special needs of Alzheimer’s and dementia related disorders.
5. Possess the ability and motivation to seek out improved methods of the delivery of resident care.
6. Must successfully complete the caregiver training class.
7. Must be free of any criminal conviction or receive criminal record waiver(s) from the appropriate licensing authority.
8. Must pass a pre-employment physical.
9. Must successfully complete a first aid training class and receive a valid certificate for first aid.
10. Must possess a valid driver's license and clean DMV record.
11. Possess the ability to deal tactfully and respectfully with facility staff, coworkers, residents, family members and prospective customers and government personnel.
12. Display courteous behavior toward family members, facility visitors, prospective customers and fellow caregivers and administrative staff.
13. Possess the ability to exercise sound judgment and make independent decisions when circumstances warrant it.
14. Demonstrate knowledge of the safe use and care of supplies and equipment.
15. Demonstrate knowledge of personal care, dietary and housekeeping procedures.
Physical
1. Continuous bending, squatting, twisting and reaching.
2. Frequent patient lifting of up to 150 pounds.
3. Repetitive grasping.
4. Fine manipulation and frequent pushing and pulling
5. Frequent prolonged standing and walking.
6. Must possess sight and hearing senses, or use adequate aids so that the requirements of this position can be fully met.
7. Must be in good general health and demonstrate emotional stability.
Job Type: Full-time
Pay: $16.00 - $18.00 per hour
