Position Overview
The Caregiver Coach will provide support, remote coaching, and education to family caregivers of loved ones (patients) living with dementia. We are looking for a highly motivated, compassionate, dedicated, and hard-working individual to support families in improving the experience of living with Alzheimer’s Disease, related dementias, and other conditions. The Caregiver Coach position is at the core of Ceresti’s innovative Caregiver Support Program, a first-of-its kind telehealth program aimed at improving the cost and quality outcomes for persons living with dementia and their caregiver.

Responsibilities
· Leverage Ceresti’s technology platform to support and empower family caregivers to provide the best possible care for a loved one living with dementia
· Educate and assist caregivers and families in understanding and adjusting to the diagnosis, treatment plan, and preventing avoidable hospitalizations
· Perform intake and onboarding of family caregivers including tablet-start trainings
· Outbound engagement with assigned caregivers to drive engagement with our program and sustain coaching interactions that drive program outcomes
· Personalize the program for caregivers as needed
· Participate in interdisciplinary care team meetings
· Prepare and maintain all pertinent records and progress notes in Ceresti’s custom-built coaching software
· Coordinate referrals to community resources
· Assist in technology-based research efforts and pilot study evaluations
· Perform all other related duties as appropriate and/or assigned
Desired Qualifications
· Bachelor’s in psychology, social work, or related psychosocial field
Required Qualifications
· Fluency in Spanish and English
Desired Experience
· Minimum of 2 years working with older adults, preferably seniors with dementia
· Direct caregiving experience (either personal or professional) with persons with cognitive impairment, family therapy, and geriatric/dementia care
· Comfortable helping caregivers work through challenging situations, providing a sympathetic and understand support system for them
· Strong ability to communicate, problem solve and work effectively in a team
· Positive and flexible attitude, willing to work in an agile environment
· Innovative and self-motivated, ability to motivate others
· Good organizational skills and ability to prioritize goals and responsibilities
· Experience working in a Health Insurance Portability and Accountability Act (HIPAA) compliant environment
· Ability to learn and understand the technology used to deliver our program (tablet/management software)
· Proficient in Microsoft Office
· Consistently transparent, courageous and enthusiastic
· Able to work independently with minimal supervision
Job Type: Full-time
Pay: $52,000.00 - $58,000.00 per year
