How You Will Make a Difference

As we expand our services in new and existing markets, we're looking for an Care Transition Coordinator who shares our values of being Trustworthy, Capable, Compassionate, Proactive, and most importantly, Called to serve patients in their home. The Care Transition Coordinator is the face of VitalCaring when interacting with referral sources. In this role, you orchestrate the perfect balance of patient care and operational efficiency, building confidence among care teams, patients and providers. As a result, your clients view VitalCaring as their preferred hospice partner that provides compassionate, personalized hospice care to their patients.

As a Care Transition Coordinator, you will:

• Contribute to a culture of accountability and teamwork
• Cultivate and nurture relationships with referral sources
• Champion communication among and between care teams
• Own individual sales goals

Experience to Deliver on our Mission

• College degree, valid state driver's license, and reliable transportation.
• One year of hospice or healthcare sales experience.
• Knowledge of business and fiscal management, governmental regulations, and accreditation standards.
