Lifespan Home Health is seeking a Care Team Assistant II to help support our growing Dallas, TX branch office.
We are an established and stable home health agency that provides in-home, medical and non-medical, care to the elderly and disabled so they can remain safe and comfortable in their home, with their family, and in the community.

Essential Tasks:
- Responsible for the receipt of home care referral and the scheduling of visits as prescribed in the plan of care.
- Will receive referrals and telephone/facsimile orders serving as the communication center between physician case manager, staff members, and client.
- Responsible for informing inquirers about the organization’s services and about home care in general.
- Schedules or reschedules visits when staff members are unable to make them as scheduled.
- Collaborates with payroll department to ensure that employee’s time records are accurate.
- Conducts client admissions/evaluations with the exception of clients in the PHC program.
-Submits required reports within the assigned time frame.
Requirements:
- High school diploma or GED required.
- Six months to one year previous staffing experience in a home health agency or other medical setting preferred.
- Travel required for client visits/hourly care, as necessary.
- Home health experience strongly preferred.
- Bilingual in Spanish preferred.
