Care Navigator

NSMHA is looking for aCare Navigatorto join our ongoing Community Mental Health Centers (CMHC) grant. This is a full time outpatient position. The Care Navigator assists with intake documentation and requirements, educates and supports individuals in accessing community resources, and aids with applications and referral documentation. The Care Navigator is a liaison with family, care providers, natural supports and recovery coaches.

What you’ll do:

· Help clients access North Suffolk’s mental health and addiction services including homeless patient population and patients with Serious Mental Illness

· Assist with enrollment coordination and documentation for CMHC Grant

· Support patients with paperwork for employment, education, housing, etc.

· Work with referring providers and community partners to coordinate client care

· Collaborate with a multidisciplinary team of social workers, counselors, nurse practitioners, psychiatrists, and administrators

· Have the opportunity to participate in organization of patient workshops, groups and team trainings

What you’ll need:

· Education: Bachelor’s degree in Social Work, Counseling Psychology, Psychology, Expressive Therapies, or related field of study

· Experience: Internship or employment experience in a multidisciplinary treatment setting
