Excelsior Wellness Center is looking for a Care Navigator to join our VIEW telehealth program.
As a Care Navigator within our VIEW program, you play a crucial role in our mission to promote mental, emotional, and physical well-being in the community. This position provides a collaborative forum to engage a variety of service partners and advocate for the needs of individuals and their families to support the family remaining together and to improve the safety and satisfaction in their lives. This role provides connection, stabilization, and support that are designed to help youth and families avoid out of home placement and facilitate warm transitions of care for their long-term needs.
How to Qualify:
- High School Diploma required. BA in counseling or related field preferred.
- Comprehensive knowledge of public systems and the ability to obtain and organize information for resources in various areas across Washington State.
- Minimum one year experience working with youth and families.
- Experience in Behavioral Health Case Management preferred.
- Exceptional communication and interpersonal skills.
- Ability to be licensed through the Washington State Department of Health as an agency-affiliated counselor.
- Must have strong multitasking and organizational skills.
- Ability to work in a fast-paced environment and make ethical and safe decisions.
- Ability to manage individual stress and perform essential job functions in a safe and professional manner.
- Must maintain a secure and reliable internet connection as this is a remote position.
- Knowledge of Microsoft Software: Teams, Word, and Excel preferred.
- Ability to pass a criminal background check, per organizational standards.
- Must be 21 years or older.
Core Responsibilities:
- Understands and actively promotes VIEW program throughout WA State, identifying ways in which efforts aimed at improving outcomes for children and families can be coordinated.
- Collaborates with VIEW team and community partners to coordinate a plan of transition for long term support needs.
- Navigator will provide case management and generate appropriate referrals to assist youth and families in reaching their treatment outcomes, such as: phone calls, telehealth appointments, meetings, and written documentation to support youth and families.
- Coordinates cross-agency communication and planning issues as needed.
- Attends and actively participates in weekly meetings, supervisions, and team staffing.
- Is aware of and available to assist VIEW and community members as needed for additional support in periods of crisis.
