Position Summary

The Care Manager Assistant provides non-clinical care coordination support functions for care coordination/case management services of members enrolled in Florida Community Care (FCC). The CMA role assists in promoting effective education, self-management support, and timely healthcare delivery to achieve optimal quality and outcomes. The Case Manager Assistant is a professional that consistently demonstrates and promotes the values of respect, honesty, care and dignity for the members as well as all other team members involved in the servicing of FCC’s clients. The Case Manager Assistant is involved in care coordination support services of dual eligible Medicare/Medicaid members and state Medicaid programs. The Case Manager Assistant works to support the care coordination model including service coordination/authorization, management and follow-up on non-direct clinical interventions. The Case Manager Assistant is responsible to assist Case Managers in the implementation of various interventions in members’ individualized care plans.


Essential Functions

  1. Using e-care database, assists Case Management teams to manage and follow up on intervention tasks received in the individualized member’s care plan and authorizes LTSS services that have been approved by Case Management teams.
  2. Contacts members as necessary to assist in coordination of services.
  3. Provides written notification to Providers on new, modifications or cancellation of LTSS services
  4. Prioritizes members according to intensity, need, and required follow-up.
  5. Provides feedback and information to Case Managers as needed.
  6. Identifies and effectively utilizes health plan benefits and community resources to meet the needs of members and caregivers.
  7. Collaborates with interdisciplinary care team and facilitates case reviews as necessary.
  8. Coordinates provision of social services, home & community based services and other identified services and resources as identified by the Case Manager.
  9. Maintains accurate and timely documentation in the electronic care coordination system.
  10. Participates in orientation of new personnel
  11. Participates in regular team meetings and department in-services
  12. Works in conjunction with others on the team and promotes collaborative teamwork
  13. Adheres to organizational policies and procedures
  14. Maintains a working knowledge of, and adheres to applicable federal/state regulations including but not limited to, laws related to patient confidentiality, release of information, and HIPAA.
  15. Maintains appropriate professional boundaries.
  16. Uses safe work practices. Promptly reports workplace and patient safety issues to supervisor.
  17. Interacts in a manner, which is professional, respectful, positive, helpful, and promotes trust.
  18. Maintains professional growth and development
  19. Complies with all requirements of the LTSS Care Management Program.
  20. Other duties as assigned.

Education and Experience

  • Associates degree in health care related field preferred.
  • Prior experience in a health care setting
  • Understanding of Medical Terminology
  • 1+ years in healthcare experience
  • Basic knowledge of care management processes
