Job Description
The Care Manager will oversee patients’ needs and coordinate care with providers and other health care professionals.
Duties and Responsibilities
- Enroll eligible patients in care management program
- Communicate with patient and their families (with consent) to develop actionable and achievable care plans
- Monitor and adjust patient care plans based on changing needs and conditions
- Coordinate referrals to health care professionals as defined by best practice protocols
Participate in interdisciplinary treatment team meetings to assure care plans meet all of patient needs
Qualifications
- Bachelor’s degree in nursing or related field preferred, or minimum 2 years of clinical experience required
- 2 years of care/case manager experience required
- Ability to collaborate effectively with physicians and other health care professionals
- Experience with EMR systems – eCW preferred
- Registered Nurse preferred.
