GOODHUE COUNTY HEALTH & HUMAN SERVICES INVITES APPLICATIONS FOR THE POSITION OF:
Care Coordinator-Public Health Nurse/Registered Nurse/Social Worker

Goodhue County Health & Human Services is seeking qualified applicants in our growing Home and Community Based Service division. We are focused on a work/life balance for all of our employees, 13 paid holidays per year, and flexibility in scheduling.
Kinds of Work:
Provide case management, care coordination and comprehensive assessments to Goodhue County residents.
Examples of Work:
Ø Complete MNChoices or South Country Health Alliance/Blue Plus comprehensive assessments in home and community settings.
Ø Assess and evaluate client's health and living situations to determine level of care and services needed.
Ø Develop person-centered care plans to address service needs in the community
Ø Collaborate with community providers to meet client needs
Ø Provides input in program planning, the development of agency goals and the evaluation of the agency's programs & policies.
Ø Manage individual budgets for approved services
Ø Maintains case file documentation.
Ø Work as part of the Home and Community based services and Health plan team.

Ø Strong writing skills with the ability to implement professional grammar and language for individual case notes, court reports and individualized support plans.
Ø Ability to establish and maintain thorough case files in compliance with state and federal laws both for ongoing case work and for audit process
Ø Strong oral skills with the ability to communicate with providers, clients and co-workers through various means.
Ø Ability to compute simple formulas while using a calculator with addition, subtractions, multiplication and division
Ø Highly skilled in sensitive mediation and negotiation with individuals, families and providers to develop support plans to ensure safety of individuals in the community.
Ø Ability to provide counseling to individuals and families on life decisions, resources and understanding information.
Ø Ability to provide advocacy, support and navigation of complex systems for highly vulnerable people
Ø Strong independent problem-solving skills using creativity, brainstorming and data analysis
Ø Ability to interpret policy and statues and apply to current work duties.
Ø Highly skilled in monitoring and budgeting for service delivery.
Ø Ability to adapt to frequent changes in environment and schedule in a fast-paced work setting
Minimum Requirements:
Bachelor's degree in nursing and certification as a public health nurse with current licensure as a registered nurse in Minnesota or current licensure as a registered nurse or a Bachelor's degree in social work, psychology, sociology or other closely related human services field. Must possess a valid driver's license. Ability to complete and pass MNCHOICES certification upon hire.
Preferred Qualifications:
One year of nursing or social work experience in home care or long-term care or experience in waiver case management.
The pay rate is $33.48/hr. Full time- 40 hours/week exempt status. The County's comprehensive benefit package includes medical, dental and vision insurance, health savings account, employer paid life insurance, short and long-term disability insurance, pre-tax flexible spending accounts, Public Employees Retirement Plan, accident insurance, tax-deferred compensation, generous vacation and sick leave and 13 paid holidays per year. County employees are eligible for the Public Service Student Loan Forgiveness (PSLF) program.
