Job Description
This position will entail working with several programs, all working with high-risk youth or adults.
Our new, Launch program, entails working with high-risk adolescents, aged 13-19, with our Launch, Youth Empowerment Program. This empowers adolescents by addressing their challenges, strengths, and mental health, while providing them with the tools and support needed to navigate their transition into adulthood. Through therapeutic guidance, mentoring, workshops, parent support, community sponsors, and collaborative partnerships, we will help teenagers develop vibrant and successful lives. This position will work 1:1 with youth, their parent/s, in a comprehensive approach to identifying their unique needs as a youth, identify barriers, create a realistic plan to help address their personal/self, relationships, educational/school-related, family/parent-child relationship issues, peer-related problems, minor to moderate mental health, independent living skills, and help them to successfully gain strength, confidence, and tools necessary to move towards and transition into adulthood. This position will entail completion of comprehensive social/emotional assessments, with the aid of a supervisor, identification of barriers, establishing goals, working closely with clients to schedule to create a partition plan of activities such as, but not limited to; support groups, workshops, receiving a youth sponsor and adult mentor, and desired level of engagement. Knowledge, experience, and skills related to mental health awareness, positive coping skills, independent living tools, trauma, care coordination, parenting education, healthy relationships, school challenges, social skills, communication, problem-solving, substance abuse and recovery, job/career/education is recommended, and community resources. Must be able to fill out assessment forms, complete needed documentation, and maintain case files. Must be able to work with diverse populations and model qualities of respect, human value, kindness, and positivity.
Our Compass Therapuetic Life Skills program entails entails working with high-risk, high-need, individuals and families, who may be dealing with crisis, safety issues, transition from incarceration, homeless or at-risk for homelessness, emotional/mental health, substance abuse, parenting problems, poverty, joblessness, or disabilities. We use a therapuetic approach to addressing mild to moderate mental health needs and holistically including life skills guidance and support to our clients.
We have a contract with Medicaid to provide FCS housing case management support to chronically homeless individuals and families.
Provide initial intake assessment for individuals/families. Establish a report of trust, value and respect with clients, to promote security and openness to engage and learn. Assess needs and help to develop goals of the individuals/families who wish to participate. Provide essential life skills, both through hands-on learning and educational directions. Assist in making appropriate agency referrals based on the needs and goals of the clients. Provide care coordination with partnering organizations who may be working with the participants and offer advocacy and assistance in compliance or other needed support for their success. Provide face-to-face care coordination services on a weekly basis per clients served, to identify barriers and goals, and to provide hands-on life skills guidance to help clients achieve these goals. Work with area businesses to establish support system for participants. Build positive support relationship of encouragement, ideas, healthy role modeling, and life stabilization aid. Services are provided both at the iMPACT! Compassion Center office and in the field/home of the clients we serve.
Requirements:
Minimum of a BA/BS in Psychology, Sociology, or similar, and/or at least 1+ years of experience in the fields of counseling, social work/case management, strong background working with youth and/or at-risk teenagers, and ability to work with a diverse clientele and special needs. Ability to communicate effectively, show strong positive-solving abilities, healthy life skills, and ability to work independently. Must be able to clear a background check and work with women and children. Must have a working vehicle and insurance coverage. Must be able to work independently with little supervision, after the training period. Must be able to demonstrate responsibility and have strong work ethics. Bilingual a plus, but not required.
