Job Summary and Qualifications

The Care Assure RN Navigator Coordinator facilitates post-discharge patient follow-up through patient coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. • You will guide patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes in connection with post-discharge care.
• You will review patient case lists to identify patients that could benefit from intervention. 
• You will serve as a point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services.

What qualifications you will need:

• Bachelor’s Degree in Nursing with graduation from an accredited school of nursing required.
• Current Nevada and California State Registered Nursing license (Apply for State Licensure within 10 days of hire date with active licensure within 90 days).
• Minimum 3-5 years clinical cardiovascular pulmonary experience (Med/Surg, ICU, ER or Cardiac Rehab) specialty area experience strongly preferred.
• Experience in program development is preferred.
• Experience as a nurse navigator and/or patient educator is preferred. Experience in leadership and customer service is preferred.
