SUMMARY

Deliver personalized, evidence-based, holistic care to stabilize patients after experiencing a crisis episode upon discharge from emergency room and inpatient psychiatric units. Connect with individuals and families with substance abuse and mental health needs upon their discharge from hospitals and provide support with therapy and care coordination services until linked to long term community providers or consumers needs are resolved.


QUALIFICATIONS

  • Master’s degree in human services
  • QMHP Clinical licensure (LSW, RN, LCPC, LCSW, LMFT. LCP)
  • Valid CADC
  • Proven experience in substance abuse treatment.
  • Expertise in evidence-based practices such as Motivational Interviewing, Cognitive Behavioral Therapy, and Dialectical Behavior Therapy.
  • Strong documentation, verbal and interpersonal skills.
  • Personal vehicle, valid driver’s license, insurable driving record and auto insurance.
  • Preferred background in a detox or residential setting.

RESPONSIBILITIES

  • Provide a continuum of counseling services to adults and youth with alcohol/drug abuse related needs.
  • Deliver services within hospital, office and community settings.
  • Maintain high quality documentation of casework consistent with outline program policies and licensure requirements.
  • Prepare written service plans to identify problem areas needs strategies and service objectives.
  • Work collaboratively with interdisciplinary and healthcare team members both internally and externally to improve patient care.
