Title
Bronson First Referral RN - Part-time 12D
Using clinical knowledge and experience, the Bronson First RN manages and coordinates the placement of all patient admissions and transfers from every point of entry within the organization during an assigned shift. Responsible for monitoring, coordinating and managing capacity both currently and prospectively in all Bronson Hospital facilities 24 hours per day/7 days per week.
EDUCATION & EXPERIENCE

  • Bachelor's degree in Nursing with 3-5 years of clinical hospital experience with progressively more responsibilities desired. Emergency Department triage, Bed Placement and/or House Supervisory experience recommended.
  • Active nursing license in the State of Michigan required.

ADDITIONAL RESPONSIBILITIES & DUTIES
  • Fluent in Microsoft Word, Excel, data base management and PowerPoint within 1 year.
  • Basic knowledge of electronic medical record systems, (i.e.: EPIC).
  • Work produces very high levels of mental/visual fatigue between 70 and 90 percent of the time, and work involving extremely close tolerances and considerable hand/eye coordination for sustained periods of time.
  • The job produces some physical demands. Typical of jobs that include regular walking, standing, stooping, bending, sitting, and some lifting of light weight objects.
  • Must possess excellent interpersonal, critical thinking, effective problem solving and prioritization skills, and the ability to process information in a confidential manner.
  • Must be able to work as a team and communicate clearly and effectively with patients, providers and other staff.
  • Has the ability to maintain patient confidentiality, work independently and think as well as function as an integral team member with other healthcare professionals.
  • Apply nursing knowledge derived from education and experience as the foundation for clinical triage communications and patient placement decisions.
  • Must demonstrate excellent customer service skills at all times and significant tact in handling sensitive matters.
  • Collaborates with physicians and operational leaders to maximize the day-to-day use of beds and facilitate patient movement.
  • Using electronic tools and dashboards, coordinates placement of admissions, regional and direct admissions, taking into account the clinical needs of the patients and competencies of the patient care delivery area as well as hospital bed availability.
  • Serves as communicator with physician offices and the region related to bed placement
  • Coordinates activities with outside health care organizations or their representatives, including communications about changes in hospital bed availability
  • Monitors Epic Dashboards in real-time to expedite and prioritize challenges with high occupancy, over-utilization of resources and administrative issues to remove barriers to discharge and address delays in care.
  • Increases effective bed capacity by identifying barriers in bed assignment processes and collaborates with house manager and clinical management in the resolution of these issues.
  • Uses advanced clinical knowledge to direct proper patient placement and utilization of beds in accordance with established standards and policies, ensuring each patient is receiving the proper level of care as per condition.
  • Regularly reviews census and acuity information to anticipate potential problems/trends and to prevent the disruption of services.
  • Reviews schedules for scheduled Surgeries, Interventional Radiology, Cardiac Cath Lab and same day surgery patients for their potential admission to ensure placement of patients, post-op / post-procedure, to appropriate levels of care in a timely manner. Works closely with these departments and knows how to prioritize bed placement using sound clinical judgment.
  • Collaborates and communicates with the local house managers, unit coordinators, charge nurses, case managers, procedural units and other core clinical service departments to manage bed requests with a plan and goal of timely patient movement.
  • Able to identify patterns requiring proactive strategies to assure patient progression.
  • Works with House Managers and Staffing to ensure needed beds are staffed accordingly.
  • Identifies clinical priorities in relation to the bed assignment process in collaboration with Physicians, ED, OR and PACU staff.
  • Provides hospital wide communication regrading level of occupancy
  • Coordinates and actively participates in daily capacity huddles.
  • Maintains knowledge of all discharges occurring throughout the hospital to accurately obtain census data in concert with Case Management.
  • Able to assist with all incidents in the hospital from a capacity management standpoint.
  • Keeps accurate records and provides analysis and assessment of more complex problems that need immediate attention from manager or administrator on call. Participates in the development and implementation of short and long-term goals for role.
