The Jim and Eleanor Randall Breast Center (managed by California Medical Business Services), provides an inviting, comfortable environment where you can receive state-of-the-art diagnostic imaging and breast screenings performed by our Board Certified Radiologists and highly skilled technologists. We are on the cutting-edge of research and have highly sub-specialized expertise in Screening and Diagnostic Mammography, Breast Ultrasound, Breast Biopsy, and Osteoporosis Screening.  Our goal is to provide high-quality diagnostic services in a comfortable, spacious, and beautifully designed environment where each patient is treated with warmth and courtesy.  Our team of professionals has extensive experience and advanced specialty training. Our skilled team of advanced licensed technologists and a dedicated clerical staff coordinate efforts to provide comprehensive care and support in a caring and professional environment. We are currently seeking an experienced, Breast Nurse Navigator join our team!

Pay Range: $45/hour - $50/hour

The schedule is Monday - Friday, 8:00 am - 5:00 pm or 7:00 am - 4:00 pm

The Breast Nurse Navigator (BNN) is a registered nurse and is responsible for the effective clinical care coordination of patients. The BNN proactively identifies and resolves needs and ensures all patients have immediate access to emotional support and clinical guidance throughout their care within the Randall Breast Center. The BNN works in collaboration with the care team and other multidisciplinary teams to establish and coordinate a plan of care across the system for The Hill Medical Corporation patients.

Your Key Responsibilities: 

  • Direct patient care, program planning and evaluation, participation in staff development and education, and quality improvement initiatives. In addition, a strong working knowledge of the specific disease care standards and core competencies is critical.
  • Will communicate diagnosis results and related information to patients and their referring physicians.
  • Will collaborate with the Radiologists, ordering physicians, and staff to determine the most appropriate care of the patient. Will perform patient care duties necessary to maintain the flow of patient care as well as coordinate the delivery of care provided by other health care team members.
  • Will maintain and support the standards of nursing care and practice to meet the healthcare needs of patients, their families, and the community.
  • Will participate in continuous quality improvement initiatives.

What You Will Do:

  • Systematically and continuously collects and assesses information related to patient health status to develop, execute, and evaluate the plan of care for patients and/or family.
  • Interact with patients, radiologists and technical staff to maintain quality of care, patient satisfaction and workflow optimization. 
  • Coordinate the patient’s plan of care with caregivers and providers.
  • Call patient after their appointment has been scheduled to complete a formal introduction.
  • Provide education about diagnosis, treatment options, side effect management, care team roles, and importance of treatment adherence.
  • Provide guidance and referrals to assist patients in coping with the diagnosis and potential outcomes.
  • Provide psychosocial support to patients, families, and caregivers, facilitating appropriate referrals as needed.
  • Assume responsibility for facilitating the scheduling of patient appointments, diagnostic testing, and procedures with appropriate scheduling team. Communicate with patient and family during inpatient stays and procedural visits.
  • Participate in Tumor Board and clinical trials, as appropriate.
  • Identify and connect patient with appropriate support services and community resources.
  • Assess educational needs of patients, families, and caregivers.
  •  Facilitate communication among members of the multidisciplinary care team.
  • Primary reporting is to the designated Radiologist and coordination of work flow related and other non-medical matters with Clinic Manager.
  • Responsible for providing medical care to patients, including assessment, treatment, care planning, and medical care evaluation.
  • Responsible for all phases of care involved with the center’s procedures, including biopsies, administration, and patient follow-up.
  • Obtains patient consent for care and ensures patient confidentiality. Obtains and documents health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition.
  • Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
  • Refers and consults with physicians, other health providers, and community resources to prevent/resolve problems or concerns.
  • Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices.
  • Participates in multidisciplinary teams to improve patient care processes and outcomes.
  • Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum.
  • Employs strategies to promote health and a safe and healthy environment.
  • Reviews patient charts, greet patients, acknowledge medical history with patient, elicit new symptoms
  • Reviews mammography and ultrasound exam results
  • Triage workflow: release normal results to patients after questions answered; for patients who need ultrasound exams – coordinate with ultrasound team as needed (as directed by radiologist)
  • For biopsies – explain procedure to patient, give benign biopsy test results to patients
  • Perform exams, mark areas of concern raised by both patient and referring physician
  • Teach normal breast exams
  • Flag difficult or unusual exam for radiologists input
  • Attends required meetings and participates in committees as requested.
  • Participates in professional development activities and maintains professional affiliations as required.
  • Maintains patient confidentiality.
  • Performs related work and other duties as required or assigned.

What You Will Bring:

  • Required License – State of California Registered Nursing (RN), ARRT (M) or ARDMS (BR).
  • Completion of nursing training from an accredited school of nursing. BSN preferred.
  • Radiology nursing experience preferred.
  • Minimum of one year of professional nursing experience. Clinic experience preferred.
  • Current CPR certificate.
  • Breast Health Clinical Navigator (BHCN) certification must be obtained after experience requirements met.
