Amazing Home Care is currently seeking a dedicated and bilingual (Spanish/English) CDPAP (Consumer Directed Personal Assistance Program) Onboarding Coordinator to join our team in the Bronx, NY. As an Onboarding Coordinator, you will play a crucial role in facilitating the enrollment process for individuals seeking home care services through the CDPAP program.

Responsibilities:

  • Conduct initial eligibility reviews and screenings for individuals interested in participating in the CDPAP program.
  • Explain program details, eligibility criteria, and benefits to potential participants and their families.
  • Coordinate and manage the intake process, including gathering necessary documentation and information.
  • Collaborate with healthcare professionals, caregivers, and clients to ensure smooth onboarding and enrollment procedures.
  • Maintain accurate and confidential records of all intake activities and participant information.
  • Provide exceptional customer service by addressing inquiries, concerns, and providing support throughout the intake process.
  • Assist in educating clients and caregivers on program guidelines and procedures.

Requirements:

  • Fluency in both English and Spanish (written and verbal) is essential.
  • Experience in a similar role in Intake or Onboarding home care clients.
  • Strong organizational skills with attention to detail.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and collaboratively in a team-oriented environment.
  • Proficiency in basic computer applications and data entry.
