Position Title:Case Aide LTFC_JOR
Reporting Relationship: Lead Case Manager
Position Summary
The Case Aide for NYAP’s La Jornada (The Journey) Program focusing on Long-Term Foster Care (LTFC) works to make a meaningful difference in the lives of the minors who have fled their country of origin and arrived in the U.S. without a parent or legal guardian. The Case Aide supports the case management staff and works closely with minors, foster families, sponsor families, and community partners to provide advocacy and support towards targeted goals.
RESPONSIBILITIES
The Case Aide will perform duties including, but not limited to:
Performing all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies.
Responsible for following the assigned intake, placement, and discharge procedures including collecting and uploading assigned documents into NYAP’s electronic health record (EHR) system and the government’s database (UCP) within the mandatory timeframe.
Writing required and additional progress notes for assigned responsibilities, and uploading progress notes and accompanying documents to NYAP’s electronic health record (EHR) system and the government’s database (UCP) within the mandatory timeframe.
Conducting two weekly telephonic contacts with family of origin, primary caregiver, and/or sponsor.
Facilitating incoming calls to minors with the appropriate family members and other approved caregivers.
Assisting with the coordination of legal service providers’ “Know Your Rights’ presentations and facilitating attorney to client contact as request by attorneys or minors.
Driving minors to program services, which may include transportation to court appointments, attorney visits, consulate interviews or other appointments.
Assisting with filing, compiling, closing, and overall daily maintenance of records found in either the physical case file and in the electronic EHR or UCP files.
Assisting with the coordination of travel arrangements for program staff transporting minors for foster placement, reunification, or transfer purposes (may include booking airfare, hotel and rental care).
On occasion, traveling with minors to help reunify with foster or sponsor families. Such travel may include out-of-state travel, airline travel, and overnight stays.
Assisting with the coordination of fingerprint services for family reunification at program designated digital site.
Consistently achieving established productivity thresholds.
Performing other duties as requested.
MINIMUM QUALIFICATIONS
High School Diploma; BA or BS Degree.
Experience in the delivery of social services in a diverse cultural population with minors including infants, children, and adolescents.
Bilingual English/Spanish fluency required.
Must have a valid driver’s license, reliable transportation, automotive insurance, and a good driving record.
Must be able to organize, navigate, and physically escort minors to various travel destinations possibly including airlines, trains, and buses. Potential travel may include out-of-state trips to unify minors with families.
A willingness to work flexible and non-traditional hours maintaining a flexible, organized and efficient work schedule and is able and willing to work extended hours and weekends.
Capacity to remain objective, professional, and effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds including refugees or other minority families.
Organized, works with a sense of urgency and is a deadline-driven self-started capable of juggling multiple priorities at once and executing each to completion despite obstacles.
Proficiency to easily listen to verbal communication in English and Spanish, and transcribe and translate that information into clear written error-free communication.
Demonstrate exceptional customer service focused on caring for people, connecting communities, and promoting peace.
Proficient use of desktop and laptop computers, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
