Job Description:
We are seeking a highly skilled and compassionate Nurse Practitioner to join our telehealth team on a part-time basis. The ideal candidate will have a strong focus on evaluating senior patients, primarily concerning their mobility and incontinence issues. This role is instrumental in providing exceptional care and support to our patients, ensuring they lead healthier, more comfortable lives.
Key Responsibilities:
- Conduct telehealth consultations with senior patients, focusing on mobility and incontinence issues.
- Assess patient needs and develop personalized care plans.
- Prescribe appropriate treatments and DME, ensuring compliance with Medicare and Medicaid coverage.
- Collaborate with a multidisciplinary team to provide comprehensive patient care.
- Maintain accurate and confidential patient records.
- Stay current with the latest healthcare practices relevant to senior care.
Requirements:
- Licensed Nurse Practitioner in New York State.
- Experience in geriatrics, primary care, or a related field preferred.
- Strong clinical skills with an emphasis on mobility and incontinence issues in seniors.
- Excellent communication and interpersonal skills.
- Proficient in telehealth technologies and electronic health record systems.
