POSITION SUMMARY: Mid-Delta Health Systems, a Federally Qualified Health Center (FQHC) has an immediate opening for a full-time position for Behavioral Health Case Manager at our clinic located in Clarendon, AR. The Behavioral Health Case Manager works as part of the Behavioral Health Team by assisting providers in their provision of Behavioral Health services. The Behavioral Health Case Manager aims to improve patient motivation by monitoring patient participation and engagement in Behavioral Health care, providing patient interventions, and screening for patient's Behavioral Health needs. MDHS is a family and Community Health Center that offers a wide range of services insured, uninsured and underinsured patients.

RESPONSIBILITIES:
Liaison between community partners, key stakeholders and health center leadership and staff

• Assists in the development of a system to assess and intervene with high-risk patients.
• Ensure SBHC services are properly integrated into all aspects of the health center functions.

Develops overall resource requirements to meet identified needs

Assists in developing and implements policies, procedures and guidelines
Utilizes current research findings to support clinical social work services best practices on an ongoing basis
Participates in the delivery of care as scheduled
Performs and documents patient's screenings, psychosocial assessments, progress and referral information
Assists in the process of advocacy for patient's access to resources through community programs and coaching to the patients, when appropriate
Work with patient and counselor to develop treatment plan based on individual needs - crisis management plans, coping mechanisms and assist in accessing resources to meet mental health needs
Collaborates and coordinates with the caregiver, medical provider, service assessment, case management, home health care and all other disciplines necessary to provide the best care to patients
Evaluate the individual needs of patients accessing mental health services
Demonstrates a commitment to service, organization values and professionalism through appropriate conduct and demeanor at all times.
Maintains confidentiality and protects sensitive data at all times.
Adheres to organizational and department specific safety standards and guidelines.
Works collaboratively and support efforts of team members

QUALIFICATIONS:
Must have the ability to work effectively independently and in a team environment

Excellent interpersonal skills
Excellent verbal and written communication skills
Requires professionalism and ability to deal effectively with patients, families, caregivers and a variety of internal and external resources
Bachelor's degree in psychology from an accredited institution preferred
Minimum of three years of experiences in a healthcare setting required
Bilingual in English/Spanish preferred
