Behavioral Health Care Manager (BHCM) - Remote

We seek a Behavioral Health Care Manager (BHCM) who is passionate about forming relationships with patients and their caregivers to improve their health and general well-being through education, care coordination, connection to community-based resources, and technology facilitation. Central to this role is to increase patients' stability and well-being in the community by fostering adherence to outpatient care and addressing social needs and drivers of utilization, thereby eliminating avoidable use of the emergency department and hospital setting.
We aim to generate a coordinated and high-functioning Health Care ecosystem to support seniors with serious mental illness and substance use disorders.
WHAT IS YOUR SUPERPOWER?

  • Develop comprehensive care plans for patients, reviewing and updating according to the review cycle and when indicated
  • Collaborate across the continuum of care with external partners supporting Behavioral Health, medical and social needs to formulate care plan and partner together for best patient care and outcomes
  • Monitor patient care plan goal progression including appointment adherence and avoidable ED/inpatient utilization, intervening as needed to eliminate barriers and close gaps in care to support appointment adherence and decrease avoidable ED/hospital visits
  • Explore benefit eligibility and coordinate benefits; ensuring patient is maximizing eligibility and benefits available
  • Supervise a Population Health Coordinator (PHC), ensuring timely follow up on care management tasks (i.e., referrals and application submission), providing guidance, and assisting navigating the healthcare system.
  • Provide crisis intervention support and direction to Community Health Workers when engaging patients in the community; guide Community Health Workers to help ensure patient safety and well-being
  • Present patients during daily interdisciplinary team huddles to discuss multiple, complex needs, formulate engagement strategies and identify interventions with owners and due dates
  • Facilitate connections between patients, caregivers, and public agencies, addressing social determinants of health with the support of a PHC
  • Document care coordination efforts in the electronic health record and relevant systems
  • Collaborate with psychiatrists, nurse practitioners, social workers, and other team members to deliver comprehensive care
  • Advocate for patients and their caregivers, offering support throughout their healthcare journey.

WHAT WE ARE SEEKING:

  • MSW or equivalent; licensed in Florida or eligible for licensure.
  • At least 2 years of experience in care coordination, case management or discharge planning
  • Proven ability to work collaboratively in a team setting.
  • Strong relationship-building skills with patients and care team members.
  • Competence in engaging patients and caregivers through various communication methods, including telephonically and telehealth platforms.
  • Knowledgeable in the differential diagnosis of common mental health and substance use disorders as well as addressing social determinants of health.

Preferred Qualifications:

  • Bilingual in Spanish-English or Haitian Creole-English.
  • Prior supervisory experience with non-clinical personnel.
  • Experience with geriatric populations and/or severe mental illness.
  • Hospital social work, case management or discharge planning experience are highly preferred.
  • Knowledge of state, and community based organizations and resources in the local market and state of Florida.

This role is fully remote. Physical requirements include the ability to use a computer. Minimal travel, up to 10% anticipated.
