Scope of Work: Minimum 18 ppd. Chart closure required in 72 hours. Mix of peds and adults.
Licenses, Certifications, Requirements: Active CA license, DEA, BLS.
The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
COVID requirement:
Primary series plus booster, regardless of date received OR
1 dose of the current COVID vaccine (2023/2024 formulation)
