- Collaborates with community partners to develop priorities and goals for various partnerships and initiatives.
- Acts as liaison between the Health Department and external partners to present and education on BASICS initiative.
- Partners with the internal health promotions team to develop outreach materials, social media messaging or digital advertising.
- Makes community presentations regarding BASICS DuPage goals and objectives.
- Seeks out grants and funding opportunities to support BASICS DuPage initiative.
- Acts as program manager for grant functions as assigned.
- Provides staff support for related advisory boards and coalitions.
- Monitors and analyzes program data and outcomes.
- Implements quality control processes.
- Develops recommendations to improve outcomes.
- Prepares regular status reports for related advisory boards and/or grant funding requirements.
- Tracks outcomes which may be submitted for research opportunities.
- Serves as the program liaison with external agencies and individuals.
- Maintains required training, licensure, and/or certification.
- Maintains confidentiality of privileged information and adheres to patient privacy laws.
- Demonstrates sensitivity and understanding of other ethnic groups and cultures.
- Demonstrates independent judgment and discretion in making position related decisions.
- Participates in emergency response activities as assigned.
Completion of a Bachelor's degree in Public Health, Education, or a related field and three years of applicable experience which included working on collaborative projects with community groups; or an equivalent combination of training and experience.
Working skill in analyzing program effectiveness and recommending improvements; working skill in group facilitation and building consensus; working skill in applying project management methods and tools; working skill in developing and maintaining positive professional relationships with internal and external customers; working skill in verbal and written communication; working skill in operating a personal computer, applicable software and peripheral equipment.Employment Type: Full Time (37.5 hours per week)
