The Clinical PharmacySpecialists are responsible and accountable for the provision of safe,effective, and prompt medication therapy. Through various assignments withinthe department, they provide support of centralized and decentralizedmedication-use systems as well as deliver optimal medication therapy topatients with a broad range of disease states. Clinical Pharmacy Specialistsproficiently provide direct patient-centered care and integrated pharmacyoperational services in a decentralized practice setting with physicians,nurses, and other hospital personnel. These clinicians are aligned with targetinterdisciplinary programs and specialty services to deliver medication therapymanagement within specialty patient care services and to ensure pharmaceuticalcare programs are appropriately integrated throughout the institution. In theseclinical roles, Clinical Pharmacy Specialists participate in all necessaryaspects of the medication-use system while providing comprehensive and individualizedpharmaceutical care to the patients in their assigned areas. Pharmaceuticalcare services include but are not limited to assessing patient needs,incorporating age and disease specific characteristics into drug therapy andpatient education, adjusting care according to patient response, and providingclinical interventions to detect, mitigate, and prevent medication adverseevents. Clinical Pharmacy Specialists serve as departmental resources andliaisons to other departments, hospital personnel, or external groups. Theyalso conduct clinical research and practice advancement projects as well aspatient care quality and regulatory compliance initiatives designed to improvemedication-use processes or pharmacy practice. Clinical Pharmacy Specialistsprovide medication and practice-related education/training and actively serveas preceptors for Doctor of Pharmacy Students and pharmacy residents. Ifapplicable, participation in the quality management program is expected withthe goal of improving services by monitoring processes, analyzing data,implementing interventions to improve and evaluating the effectiveness of thoseinterventions. Responsibilities may includeworking to establish and maintain long- and short-term goals for the Quality ManagementProgram; monitoring and documenting Quality Improvement Projects for progressin meeting QI goals; and providing guidance and education to staff on QualityManagement priorities and projects.
Tuba City Regional Health Care Corporation, a JCAHO accredited health center, provides services to a 4,400 square mile area and serves as a referral center for the western part of the Navajo and Hopi Reservations. The hospital was constructed in 1975 with a bed capacity of 73. Services provided to the community include comprehensive inpatient/outpatient emergency services, dental and ophthalmology services, orthopedics, OB/GYN, oral surgery, eye surgery and urological procedures. Tuba City Regional Health Care Corporation is located in Northwestern Arizona on the land known as the Colorado Plateau. This semi-desert landscape varies in elevation from 4,400 in the Little Colorado River Valley to nearly 7,000 feet at Preston Mesa. The staff of medical providers includes surgeons, general medical officers, internal medicine, psychiatrists, pediatricians, family practice physicians, obstetricians, ENT specialists, orthopedic surgeons, and urologists. Dental staff includes general dentists, endodontists, oral and maxillofacial surgeons, pediatric dentists, dental assistants and hygienists. The nursing staff includes registered nurses, licensed practical nurses, nursing assistants, community health nurses, nurse practitioners, nurse midwives, CRNA, and community health medics.
** Fingerprinting must be submitted and cleared prior to start. ** REQUIRED: Doctor of Pharmacy; Graduate ofan ACPE accredited College of PharmacyAND Completion of ASHP accredited PGY-1 Pharmacy Residency OR three yearsexperience as an inpatient clinical pharmacist (with at least one yearin a critical care setting (ICU,CCU, or emergency room). Must have a current valid and unrestricted license topractice as a pharmacist in a State or Territory of the United States, or theDistrict of Columbia. Must have and maintain a current valid BasicLife Support (BLS) from the American Heart Association
