POSITION TITLE: Unlicensed Personnel

DEPARTMENT: 24-Hour Customized Living

SUPERVISOR: RN Supervisor

JOB SUMMARY:

Employees in this position are responsible for providing personal care and designated health-related services, designed to maintain a client’s physical and emotional well-being, for clients living in the Belevo Health Services facilities. This position assists clients with the tasks of daily living through services indicated on the Service Agreement, the Tasks Assigned, the Behavior Plan, and other services as delegated by the RN.

QUALIFICATIONS:

  • High School Diploma or equivalent
  • Must be 18 years of age or older
  • Current and valid Driver’s License, automobile insurance, and a dependable vehicle to transport clients
  • At least 2 years experience with home health care services and mental health

SPECIALIZED KNOWLEDGE AND ABILITIES:

This position requires:

  • Sensitivity and aptitude for working with individuals with mental illness and/or chemical dependencies.
  • Prioritizing and organizing work effectively and efficiently.
  • Skills to communicate effectively with clients, families, staff, and other individuals.
  • Read, write and comprehend simple instructions, correspondence and documentation (including medical terminology), in English.
  • Puncuality and a good attendance record
  • Strong interpersonal skills.
  • A basic understanding of how to use modern technology.

ESSENTIAL JOB FUNCTIONS AND TASKS:

Job Function: Medication Administration

Tasks:

  • Medication reminders and administration as required according to the Medication Administration Record (MAR) and as delegated by the RN Supervisor.
  • Follow policy for administrating PRN/Standing order medications.
  • Follow policy and procedure for narcotic administration and documentation.
  • Follow policy and procedure for medication administration when a client takes a leave of absence.
  • Follow policy and procedures when receiving new or changed medication orders.
  • Reorder medications and supplies weekly, or as needed.
  • Understand standard precautions used for infection control, and adhere to them.
  • Other medication administration duties as assigned by the RN Supervisor.

Job Function:Providing Personal and Direct Care to Clients

Tasks:

· Assist clients with activities of daily living as indicated on their customized service plan (including, but not limited to dressing, showering, grooming, toilet assistance, and transportation).

· Assist clients with housekeeping duties.

· Assist clients with personal laundry duties.

· Assist clients with meal preparation and eating.

· Maintain a clean and healthy environment.

· Promote independence, not dependence, with clients; offering choices and fostering self-help skills.

· Perform routine procedures as delegated by the RN Supervisor.

Job Function: Documentation and Reporting

Tasks:

  • Document any changes in clients’ condition in the client chart and communication log, report these changes to the RN Supervisor.
  • Completes incident reports according to policy.
  • Report to RN Supervisor any medication changes and errors, or any PRN medications that have been administered to a client.
  • Understand procedures regarding charting and documentation of completed assigned duties.
  • Recognizing and report abnormal signs and symptoms, or changes in clients’ condition.

Job Function: Completing Essential Paperwork

Tasks:

· Filing papers in client files, as needed.

· Signing off on the Medication Administration Records (MARs) after each medication administration.

· Review all new client Service Plans, Tasks Assigned, Behavior Plans, and changes in clients’ plan of care.

· Review the communication log/shift notes on a regular basis.

Job Function: Knowledge and Compliance with State/Federal Regulations

Tasks:

· Understand and adhere to the Vulnerable Adult and Abuse policy, Homecare Bill of Rights, and HIPPA.

· Work to ensure facility is in compliance with federal, state, and local standards and regulations.

· Complete a minimum of eight (8) hours of documented training.

· Be up to date with completing Tuburcelosis testing.

Job Function: Activity Programs

Tasks:

  • Encourage clients to participate in scheduled activity programs.
  • Lead activity programs as assigned.
  • Socialize with clients.
  • Incorporate new ideas and client suggestions into activity programs.

Job Function: Participate and Function Effectively as a Team Member

Tasks:

· Develop and maintain a positive working relationship with co-workers. Provide back-up to other staff as needed to ensure 24 hours staffing.

· Communicate information regarding client care needs and changes to the client’s care team.

· Provide proactive and constructive participation in meetings.

· Be compassionate, work with professionalism, and remain ethicallty aware.

· Be flexible and adaptable to changing situations.

· Respond appropriately to safety hazards or emergency situations.

· Follow through promptly on assigned duties.

· Assist with on-the-job training for new associates.

· Remain calm in difficult or unusual circumstances.

Work Environment:
