- Responsible for the competency, capacity, and compliance of direct reports.
- Provide direct interaction daily with team members to ensure delivery of services are met for the participants utilizing best practice.
- Collaborates with Interdisciplinary Team (IDT) and other leaders to establish and maintain consistent care delivery for participants.
- Fielding and following up on grievances.
- Plan, organize, develop, implement, and interpret Senior Care Partners P.A.C.E. goals, objectives, policies, and procedures, and carry out for Senior Care Partners PACE.
- Support Center Manager in all IDT activities.
- Provide support and address concerns from vendors, including residential facilities.
- Conduct participant care team and IDT meetings as needed.
- Responsible for developing and supporting company initiatives and goals.
- Drives inclusion & employee engagement within team.
- Assist with the recruitment, interviewing and the selection process for open positions.
- Coaching, mentoring, and developing direct reports.
- Managing time and attendance for direct reports.
- Responsible for performance management of direct reports.
- Must complete all required education and file requirements on time.
- Keeps the leader informed of problems or issues.
- Performs other duties as assigned.
- PT, PTA, OT, COTA, RT, RTA and RD.
- Bachelor’s Degree is required or any combination of education and related work experience.
- Minimum of two years’ leadership experience required; or equivalent combination of education and applicable experience.
- Detail oriented with good problem-solving skills.
- Effective communication skills, both written and verbal.
- Demonstrates leadership principles throughout the department.
- Demonstrates ability to effectively facilitate meetings.
- Demonstrates ability to effectively build team cohesion.
