Job description

SIS Certified Assessor Temp-to-hire

As a SIS (Supports Intensity Scale) Certified Assessor, you will be conducting assessments for persons with disabilities to ensure they receive much-needed services within the community. Persons being assessed reside in the community or in facilities. You may also complete review decisions regarding services needed to be successful in the community and will conduct training of new assessors and quality assurance competency reviews as assigned.

This role is anticipated to start in February, starting at 4 months with the potential to hire full-time.

Region:
Northern Virginia: Fairfax, Arlington, Alexandria, Springfield
Western Virginia: Winchester, Front Royal
Work Location: Home-based with local travel
Hours: Will work between the hours of 8:00 am to 5:00 pm

Essential Functions

  • You will perform face-to-face assessments of persons with disabilities. You will collaborate with healthcare providers and ancillary service providers to create or validate the appropriate treatment plan.
  • You will document assessment results for the individual member status or negotiate a service plan as needed.
  • You will complete quality assurance competency reviews and other tasks/projects as assigned such as new assessor training.
  • You will provide ongoing education and may communicate with all healthcare providers to ensure the appropriateness of care.

Requirements

  • Two or Four-year degree in a health care or related field.
  • Case Management experience preferred.
  • At least one year of experience completing level-of-need assessments is preferred.
  • Active contract-specific required certifications for the assessments and/or quality assurance and training activities being completed.
  • Must pass and maintain SIS Certification - Telligen will provide.
  • Minimum of 2-4 years working with individuals who have disabilities.
  • We are unable to consider candidates outside of the United States, candidates in the state of California or those in US Minor outlying islands and territories.
