SalaryAs an Assessor Reviewer, you will be responsible for face-to-face iRAI and possibly SIS assessments for Medicaid members on or applying for waivers. Persons being assessed reside in the community or facilities. Assessor Reviewers also may complete review decisions regarding services needed to be successful in the community.
Hours: 8:00 AM -4:30 PM CST; May have to work later on occasion if a member requests a late assessment.
Essential Functions
- Perform face-to-face assessments of persons with disabilities.
- Some contracts may require collaborating with healthcare providers and ancillary service providers to create or validate the appropriate treatment plan.
- Document assessment results for the individual member status or negotiate service plan as needed.
- Provides ongoing education.
- May communicate with all health care providers to ensure appropriateness of care.
Requirements
- Must have either a four-year degree or four years of professional experience in the field of intellectual, developmental, or physical disabilities.
- Case management experience preferred.
- Ability to travel to cover the assigned geographic area.
- Experience in human services and working with individuals who have disabilities is strongly preferred.
- Previous experience completing interRAI and/or SIS assessments preferred.
