GENERAL SUMMARY:
This position is responsible for assisting physicians/providers by performing Medicare Annual Wellness Visits on eligible patients. This role requires travel throughout Central Florida.

PRINCIPLE DUTIES AND JOB RESPONSIBILITIES :
• Provides care appropriate to adult and geriatric patients, including knowledge of growth and development
• Obtains and interprets information to identify patient needs and provide care needed
• Provides interventions within the scope of license that have direct influence on clinical and financial outcomes
• Collaborates with team members/stakeholders to provide a smooth, organized and efficient flow of patients in a systematic approach to provide and coordinate health care services to a defined population across the continuum
• Performs billable services such as Annual Wellness
• Functions as a liaison between patients, physicians/providers and staff to provide accurate communication.
• Performs Nurse Visits by protocol
• Performs Medicare Annual Wellness Visit:
o Measure height, weight, BMI, BP and other routine measurements
o Review Health Risk Assessment (HRA) and address concerns/questions
o Provides care coordination and provides written/typed action plan for preventive care and screenings
o Complete medication reconciliation
o Provides patient education and wellness counseling in accordance with guidelines
o Ensure all appropriate documentation is entered and uploaded to electronic medical record (EMR)
o Satisfy Quality Measures in accordance to FHMG/AdventHealth strategic plan

• Ability to work with supervising physician to initiate care or consultation when appropriate
• Capacity to collaborate with professional colleagues as necessary to ensure quality care
• Able to interpret, adapt and apply guidelines and procedures
• Skilled at preparing and maintaining records, and responding to correspondence
• Able to react calmly and effectively in emergent situations
• Present with an authentic, friendly, welcoming and professional public facing appearance
• Ability to effectively provide instruction, coaching and mentoring
• Excellent communication skills; both verbal and written
• Outstanding organizational skills

• Strong computer skills including previous experience with an EMR and Microsoft Office

EDUCATION AND EXPERIENCE REQUIRED:
• Graduate of an accredited school of nursing
EDUCATION AND EXPERIENCE PREFERRED:
• 3-5 years of experience as a clinical nurse or nurse educator
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
• LPN License
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
• RN License
• BLS Certification
