SAH ANTICOAGULATION CLINIC

Full Time · Day Shift · Monday-Friday 8-5pm

POSITION SUMMARY:
The Ambulatory Care Pharmacists at UW Health Northern Illinois is responsible for providing patient care services, in collaboration with referring physicians, for disease states including but not limited to: anticoagulation management, hypertension, diabetes, smoking cessation, pain management, and congestive heart failure. The incumbent is responsible for comprehensive care including ordering and interpreting labs, ordering and refilling prescription medications, monitoring patients for adverse outcomes, providing disease state education, compliance counseling and other activities required to optimize clinical outcomes. The clinician will maintain competency in utilization of point-of-care testing devices and monitoring equipment as indicated for disease state management. The Pharmacist provides oversight of student and resident pharmacists as assigned.

EDUCATION/TRAINING:
Bachelor’s degree in Pharmacy or a PharmD Degree required. Completion of an ASHP-accredited residency training program is preferred.

LICENSURE/CERTIFICATION:
Current Pharmacist license from the State of Illinois or must obtain within 90 days of hire date.

EXPERIENCE/SKILLS/ABILITIES:
Successful completion of annual clinical, age and job specific competencies and skill verification tools required. Good interpersonal, organizational and critical decision-making skills required. Ability to use calculator and computer. Ability to effectively communicate and develop collaborative working relationships with patients, medical staff and the public.
