Description

Position: Aftercare-Family Focus Therapist I

Salary: 42,000- 52,000 

 (Starting salary depends on education, licensure, and years of experience)

Job Type: Full Time Salaried Exempt

Purpose:

The Aftercare Therapist provides a hybrid of in-home and telehealth therapy services, including individual, and family in addition to crisis management as needed.  The Aftercare Therapist will be part of a collaborative, supportive, close-knit team that works with youth and families. The service provider provides therapeutic services to help keep families together, heal developmental trauma, encourage hope and create lasting change for families. The Aftercare Therapist  practices a trauma informed and evidence based model that engages with families by demonstrating immediate responsiveness to their needs and maintaining strength based relational focus. This candidate will be able to hold a hybrid of in-home and virtual flexible work schedule while meeting needs of position. Services will primarily be focused at home and in the community with in-office and telehealth available if needed. This position includes traveling to clients’ home and communities.

Essential Responsibilities

  • Diagnoses client’s problems by collecting and evaluating information about the client’s situation.
  • Provide in-home and telehealth therapy to children and families within a 50-mile radius.
  • Maintain an average client case load of 8-10 youth.
  • Develops treatment plan by establishing treatment goals with the client; determining treatment methodologies.
  • Advises client by suggesting and exploring resolutions; discussing progress toward goals.
  • Obtains services by initiating referrals.
  • Monitors progress toward treatment goals by evaluating and adjusting services provided.
  • Assures quality service for clients by enforcing rules; regulations, and legal requirements with clients; documenting events of the therapeutic process in a confidential manner.
  • Maintains agency credibility by establishing working relationships with sponsoring, advisory, and related service agencies.
  • Promotes the agency by ensuring an understanding of program services available for

clients; publicizing activities and accomplishments; adhering to a professional code of

ethics.

  • Maintains professional and technical knowledge by participating in workshops; reviewing professional publications; establishing personal networks.
  • Provide a clinical experience for interns by negotiating learning objectives; supervising practice and caseload management; teaching theory and its application; evaluating skills and personal and professional growth; ensuring identification with the profession.
  • Contributes to team effort by consulting with other professionals; accomplishing related results as needed.

 

Required Knowledge

  • Computer literate and high knowledge of various computer software programs.
  • Therapeutic techniques and knowledge of community resources.
  • Knowledge of child development, diagnosis, and being trauma-informed.
  • Experience and acceptance of working with diverse children and families.

 

Required Skills or Abilities

  • Ability to engage clients and families.
  • High level of verbal/writing skills.
  • Treatment and discharge planning.
  • Diagnosis and risk assessment.

 

Minimum Requirements

Education Level: Master’s degree                            

Other minimum requirements: Professional licensure (e.g. LMSW, PLPC)

Years of Experience: 1 to 3 years

In addition to the above requirements, incumbent must:

  • Complete Sanctuary training in a timely manner;
  • Have a valid driver's license and proof of current vehicle insurance;
  • Follow all agency policies;
  • Be able to pass State Abuse, Neglect and criminal background checks;
  • Be able to carry out the essential functions of the position with, or without, accommodation. 
  • Perform all job duties in a manner which demonstrates a commitment to Sanctuary values and practices.
  • Demonstrate awareness and understanding of cultural and ethnic diversity of clients, staff and other community partners.

 

Physical Requirements

  • Ability to perform a range of physical motions including, but not limited to:
  • Lifting and carrying up to 50 lbs
  • Support and safely lower 150 pounds of weight to the floor
  • Standing, walking, sitting for long periods of time
  • Must be able to get up and down off the floor unassisted
  • Must be able to reach above their heads, bend, kneel, and lift
  • Running for brief periods of time
  • Must possess, or be willing to attain CPR, First Aid, and Safe Crisis Management (SCM) Training (with sufficient passing score) within 2 weeks of hire date.
