Job description

The Afghan Refugee Patient Care Advocate - Population Health is responsible for working directly with patients, providers, emergency room, clinical inpatient and outPatient Care teams, pharmacy, as well as social work, case management, home health, skilled nursing facility partners, inpatient rehab facility partners, and local, state and federal resource providers in supporting the management and coordination of Afghan Refugee patients being followed by the International Family Medicine Clinic.
The UVA Health Population Health department strives to improve the patient experience, the quality of care, and outcomes while managing costs for defined groups of people, as well as to bring significant health concerns into focus so that issues can be addressed by matching needs to resource allocation as a means to overcome the barriers to healthcare that drive poor health conditions. Population Health seeks opportunities to collaborate internally and with other agencies and organizations to improve the health outcomes within the local, regional, and state areas to care best for the patient populations served.
This position serves a diverse patient population, including many Dari and Pashto-speaking patients.
Preference will be given to applicants who are multilingual and fluent in both Dari and Pashto.
Manages a panel of patients and communicates regularly with these patients to follow up on progress and review/discuss barriers or challenges to health care, environment, patient experience, or any flagged SDoH need, as well as potential adjustments to any identified need. This information is then shared with providers, clinical care teams, and associated agencies of care.
Communicates with providers, clinical team members, the International Rescue Committee (IRC,) and other agencies involved in the patient's overall care and participates in the initial enrollment and assessment.
Schedules patients for medical appointments and appointments with the International Rescue Committee (IRC.)
Coordinates transportation for appointments utilizing various resources, non-profits, and agencies.
Reviews and maintains charts and facilitates patient accessibility to this information.
Reviews and understands patient assessments, Individualized Care Plans, Discharge Transition Plans, patient discharge summaries, After Visit Summaries.
Collaborates with teams across multiple clinics and areas.
In addition to the above job responsibilities, other duties may be assigned.
Position Compensation Range: $16.31 - $25.28 Hourly
MINIMUM REQUIREMENTS
Education: Bachelor's Degree preferred, but also acceptable is an Associate's Degree in Medical Assisting from an accredited Program of Study such as the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accrediting Bureau of Health Education Schools (ABHES) or Graduation from a certified Paramedic or EMT-Intermediate education program or Licensed Practical Nurse (LPN) from a state-approved nursing program with a minimum of 400 hours of direct client care across the lifespan and passed the NCLEX-PN exam to show proficiency.
Experience: 1 year experience in a clinical environment and/or in an advocate or navigator role preferred.
Licensure:(Only required if Candidate does not have a bachelor's degree) If MA Certified Medical Assistant (CMA) by the AAMA (American Association of Medical Assistants) or Registered Medical
Assistant by the AMT (American Medical Technologists) is required.
If EMT, certified as an EMT-P or EMT-I in the Commonwealth of Virginia or certified as a NREMT-I or NREMT-P (National Registry of Emergency Medical Technicians) required. If LPN, active licensure through the state of Virginia is required
Language: Not required, but preference will be given to applicants who are multilingual and fluent in Dari and Pashto.
PHYSICAL DEMANDS
Job requires standing for prolonged periods and frequently bending/stooping/traveling. Proficient communicative, auditory, and visual skills; Attention to detail, ability to write legibly, basic computer skills, ability to learn, document in, and communicate via EMR.
