Your Specific Responsibilities:

 

This is advanced, specialized, and highly responsible public health nursing position with responsibilities to include planning, directing, monitoring, and evaluating the clinical health services at the Florida Department of Health Gadsden County.  The APRN in this position works under the direct supervision of the Community Health Nursing Director providing direct client services and fulfilling Medicaid, Federal Title X, and DOH-Gadsden Standard Operating Procedure requirements conforming with national standards of care. 

 

Duties are as follows:

 

Provide clinical management and direct services of clients in the Family Planning, Primary Care, Improved Pregnancy Outcome, Immunization, and Sexually Transmitted Infection clinics.  Provides documentation and review in the electronic health record. 

 

  • Provides comprehensive assessments for well child and adult development, school entry and Medicaid screenings utilizing various techniques such as observation, palpation, percussion, and auscultation. Compile, interpret, and evaluates medical histories, physical findings, laboratory results and if necessary, initiate further diagnostic testing or consultation. 

 

  • Performs examinations on Family Planning and FBCCP clients as appropriate according to national and local guidelines.  Obtain pap smears, GC and chlamydia testing, wet mount, and provides prescriptions as indicated.  Communicates with clients as exam progresses. Interprets physician instructions with appropriate referrals for abnormalities to PMD or specialty care.  Performs, orders, and interprets routine laboratory tests including wet mounts, STI testing, pap smears, mammograms, and routine blood work as appropriate.  Initiates and adjusts contraception plans and device, obtains informed consent with emphasis on pertinent aspects and safety of birth control options chosen by client.  This includes LARCs, OCPs, non-hormonal methods, and sterilization options.  Infertility counseling is included in our clinics.  Provides clients with health education and counseling including psychosocial dimensions in the areas of nutrition to include but not limited to healthy weight, diabetes, and hypertension.  Counseling also includes reproductive life plans, contraceptive methods, and STI prevention.

 

  • Monitors Healthcare Effectiveness Data and Information Set (HEDIS) report and measures on all assigned panels of primary care clients.  Monitors medication and initiates appropriate therapies for medical conditions and alters medications according to variations in patient’s response within established protocols.  Enters all findings and services provided in a systematic, accurate, and succinct form into the electronic health record in a timely manner.

 

  • Provide consultation to epidemiologists and nursing staff in disease control. Prescribe prophylactic antibiotics to case contacts for certain reportable diseases.  Assist with case interview and reporting case information in the MERLIN system.   Assist with the coordination of care with HIV/AIDS clients which includes initiating ART through the FDOH test and treat program. Also, initiate PrEP and PEP therapy on high-risk clientele.  Assist in the clinical management of patients with active tuberculosis, suspected tuberculosis, or latent tuberculosis infection. Work closely with TB nurse case managers and prescribe medication and diagnostic testing as necessary. 

 

  • Precepts both DOH-Registered Nurses for STD Credentialing and APRN student nurses for family practice placements

  • Skill in performing uncomplicated surgical procedures.
  • Ability to administer prescribed medications.
  • Ability to perform complete physical appraisals of patients.
  • Ability to identify and evaluate illnesses.
  • Ability to initiate diagnostic laboratory tests and evaluate the results.
  • Ability to manage the care of patients.
  • Ability to work with patients having a variety of physical problems.
  • Ability to compile and evaluate medical histories and other clinical and laboratory data.
  • Ability to plan, organize and coordinate work assignments.
  • Ability to communicate effectively.
  • Ability to establish and maintain effective working relationships with others.

 

Qualifications:

Certification as an Advanced Practice Registered Nurse in accordance with Florida Statute 464.
