The APP, Advanced Practice Provider, provides clinical care to patients through telehealth, including diagnosing, treating, and consulting as part of the patients’ clinical team.
RESPONSIBILITIES
- Provide telehealth visits to patients enrolled in program.
- Conducts comprehensive patient assessments.
- Diagnose and treat acute and chronic illness, injuries, and health conditions.
- Prescribes appropriate ordering of medications, services, and equipment.
- Collaborate with physicians, nurses, and other health care professionals to ensure comprehensive care.
- Provide ongoing management throughout the day on assigned patients.
- Coordinating and care management from other disciplines (consultants, palliative care, social work, etc.).
- Conducts patient education when working to manage chronic illness or wellness care.
- Refers for specialty care when needed.
- Opportunity to assist with new program pilots and workgroups.
- Consult Provider/Case Finding:
- Consult provider– answer secure chats from hospital service lines (inpatient, RNCCs, liaisons, etc.) and HHC liaisons regarding patients who are potential/questionable candidates for HHC while on service.
- Case Finding—When needed, providers may be asked to help case find to identify potential patients for HHC.
- After hours call (5pm - 7am) as needed
EDUCATION REQUIREMENTS:
- Master's degree or higher with completion of accredited Nurse Practitioner program or Physician Assistant Program required.
WORK EXPERIENCE REQUIREMENTS:
- Minimum of 2 to 5 years of experience as a Nurse Practitioner/Physician Assistant
- Able to assess, diagnosis, and treat acute conditions
CERTIFICATION REQUIREMENTS:
- Certified Nurse Practitioner: American Nurse Credentialing Center (ANCC) or American academy of Nurse Practitioners (AANP) required certification.
- Certified Physician Assistant- National Commission of Certification of Physician Assistants (NCCPA).
- Licensed Registered Nurse - Board of Nursing required upon hire (NP’s).
PHYSICAL REQUIREMENTS:
- While performing the duties of this job, the employee may be regularly required to stand, sit, talk, hear, and use hands and fingers to operate a computer, telephone, and keyboard.
- Specific vision abilities required by this job include close vision requirements due to computer work.
- Ability to sit at a computer for an extended period.
Salary Range: $110,000 - $125,000. Individual compensation is based on skill set, experience, qualifications, location, labor-markets, and other job-related reasons.
General Work Hours: 7am-5pm CST
