SalaryOptimum Health is a mobile medical clinic currently composed of 5 Physicians, 5 Advanced Practice Providers, 1 Pharmacist, and 4 Medical Assistants. We are growing rapidly and are looking to hire an Optimum Health Primary CareAdvanced Practice Provider who lives, or would like to live, in eastern Idaho.
You would work with our other Optimum Health Providers treating patients in skilled nursing facilities, assisted living facilities, and patient's homes.
As an Optimum Health Primary CareAdvanced Practice Provider (APP), you would have the following responsibilities:
- Serving as patients Primary Care provider
- Evaluating and treating patients in skilled nursing facilities, assisted living facilities, and their private homes
- Work closely with your supervising Optimum Health Physician to provide efficient, comprehensive care at the highest standards
- Perform visit encounters for: acute complaints, chronic care management, remote patient monitoring, discharge planning, transitional care, and more.
- Work in close collaboration with all our providers to provide integrated, comprehensive medical care
As an Optimum Health Primary CareAdvanced Practice Provider, you would have the following flexibility:
- You would set your own schedule
- Our Medical Assistants create your personalized schedule of encounters
- Our Medical Assistants create and prep your charts for all your encounters eliminating any transcription level tasks
- No holidays, nights or weekends required
