Advanced Practice Provider - Nurse Practitioner - Transplant Surgery

PRIMARY FUNCTION :
To perform, under the responsibility and supervision of the surgeon, diagnostics and therapeutic tasks to allow the physician to extend his/her services through the more effective use of his/her knowledge in the sphere of decision making required to establish a diagnosis and to plan therapy.. Additionally, the Transplant APP will act as the patient advocate in the medical care systems serving as confidant, advisor, and liaison between patients and physicians. Reports directly to the surgeon for all patient-care-related functions.
ESSENTIAL FUNCTIONS:

• Provide clinical support in an inpatient and outpatient setting.

• Provides quality, complete and cost-effective Advanced Practice care for patients, families, and others in the transplant surgical and medical population and participates in the surgical and medical management of patients to achieve and/or maintain optimal health status, manage disease and reduce risk of illness.
• Duties will include, patient management and crisis intervention on the surgical and critical care units primarily including, but not limited to, the pre and post kidney and liver transplant and end stage organ failure patient population, as well as discharge planning, and in-patient and emergency department consultations.
• The position also requires handling after-hours patient calls and triaging the calls as indicated.
• Interacts with co-workers, visitors, and other staff consistent with the iSCORE values of Jefferson.

OTHER FUNCTIONS AND COMPETENCIES needed in addition to Essential Functions:

• Performs and records comprehensive history and physical examination.
• Executes/relays medical regimens as directed by the supervising physician and/or as described in the work agreement between the physician assistant and the supervising physician.
• Writes roster, admission orders, and arranges for labs, studies, etc.
• Assists the supervising physician in clinic and with inpatient rounding
• Provides ongoing comprehensive outpatient services as directed by the supervising physician.
• Participates in the postoperative care of patients, being especially knowledgeable in the recognition of postoperative complications, specifically:
  • Informs the physician of all changes in the patient's ongoing condition
  • Collects ongoing data and write pertinent notes on patient's chart on a daily basis.
  • Assesses the patient's postoperative course and implement indicated therapy.
  • Summarizes details of patient's hospitalization, including operative procedures and follow-up of complications.
  • Makes routine rounds as directed by the surgeon.
  • Assists the supervising physician with correspondence relevant to the patient's care.
  • Attends appropriate in-house conferences and attend continuing medical education.
  • Examines preoperative and follow-up patients in the clinic prior to their being seen by the surgeons and complete admission formalities.
  • Participates in clinical research, review of patient records and selected animal projects
  • Maintains ongoing departmental quality improvement studies in concert with supervising physician and director of physician extender services.
  • Fully utilize and maintain training with the institutions medial record system.
  • Maintains CEU's, and membership in a professional organization
  • Other duties assigned.

Qualifications
EDUCATIONAL/TRAINING REQUIREMENTS:
BS and Graduate of accredited program
CERTIFICATES, LICENSES, AND REGISTRATION:
National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant; must hold PA medical license and DEA. BLS certification required
EXPERIENCE REQUIREMENTS :
Preferred 5+ years in a clinical setting
ADDITIONAL INFORMATION:
Variable hours of duty (i.e., day, evening, night and weekend shifts) may be necessary. Accountability to the patient, family, nursing and medical colleagues for expert knowledge, judgment, and/or direct patient care.
