In this role, you will work closely with the clinical education, learning and market implementation teams under the CenterWell Primary Care Organization. Collaboratively with market leadership, you will train primary care providers in quality-based, seniorfocused medicine for existing and new centers in the PCO. As a provider trainer, you will be the ambassador for value-based, senior care model in the primary care network.

Use your skills to make an impact
 

The main responsibilities of the role are:

•    Develop, plan, organize, and supervise training for primary care providers including principles of population health, value-based care, senior care. May also need to incorporate EMR, compliance, primary care policy and procedure, coding and documentation, quality, utilization, customer service standards, team communication as needed. Training could occur in-person in markets and virtually through live and recorded sessions.
•    Serve as faculty for provider onboarding program.
•    Monitor medical performance through direct observation, chart review, data review, and provide guidance to ensure that the quality of care being provided meets appropriate standards and to ensure cost-effective utilization practices.
•    Work collaboratively with implementation teams, compliance, market teams and PCO leadership to identify training needs for providers.
•    Represent the organization in recruiting and marketing events as needed.
•    Maintain active clinical practice in the local market for a portion of the time. This is currently set to 20%. 

Education/Credentials/Experience:

•    Graduate of accredited NP or PA program with certification in Family Medicine, Internal Medicine or Geriatric Medicine with continued certification throughout employment.
•    Maintain license in state of practice.
•    Experience supporting and facilitating training initiatives for providers.
•    Clinical and operational leadership experience in the areas of Population Health Management, Practice Management, (VBC) Value Based Care and/or healthcare operations setting, preferably in Medicare Advantage programs. 
•    Strong knowledge of MA coding and documentation policies and procedures in a value-based care model, prior teaching experience preferred.
•    Basic computer skills:  Microsoft Office applications, Word and Excel, email, apps, and navigating the EMR.  eCW experience is preferred. 
•    Ability to travel up to 30%, largely within local and regional markets.

•    Communication: You actively listen to others to understand their perspective and ensure continuous understanding regardless of communication channel or audience.

Reporting Relationships
You will report to the National Director, Clinical Education. This area is under the leadership of Humana’s Primary Care Organization- Chief Transformation Officer.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

 

$102,200 – $140,700 per year

 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
