The Admissions RN/Hospital Liaison has a weekday schedule, offering 5-8/hour shifts or 4-10/hour shifts, with rotating weekends that comes with a substantial differential. There is flexibility in arranging your daily schedule, which provides a work/life balance allowing time for personal needs.
SUMMARY OF JOB RESPONSIBILITIES:
- Responsible for providing information regarding HopeWest services and community resources to individuals with serious illness and their identified support systems.
- Assists patients and their families in determining what services best meet their needs and for what services they may qualify.
- Primary representative and resource for hospital discharge planners the Admissions RN/Hospital Liaison coordinates and completes the referral-admission process in the home, hospital and facility setting.
- Provides education to the community regarding the variety of services provided by HopeWest through the different programs as well as educating HopeWest staff on referral and admission processes.
Who Should Apply?
- RN’s with two to three years of hospital nursing experience, or comparable nursing experience.
- RN’s with strong communication skills and a desire to work in a team environment
Requirements, Pay and Benefits:
- Must have unrestricted RN license in the State of Colorado
- Must have valid Colorado driver’s license, reliable transportation and auto insurance coverage
- Starting hourly pay: $35.00 - $36.50 (plus applicable differentials for weekend coverage)
- Premium pay for BSN and bilingual fluency
- Mileage reimbursement of 60cents/mile for work related driving
