Admissions Registered Nurse

McAngel Home Visiting Nurses is looking for a Start of Care Registered Nurse. The RN must have home health experience (within the last 5 years) including some proficiency with filling out the OASIS assessment form. The Registered Nurse would be responsible with Start of cares, Resumption of cares and Recertifications.

This job position would entail contract hours per week.

Responsibilities:

• Assessment of patient and conditions as per homecare guidelines
• Collaborate with physicians and nurses to devise individualized care plans for patients
• Perform Start of care visits, recerts, resumptions and discharges.
