Christian Care Communities is currently seeking an Admissions Nurse LPN or RN for our Christian Health Center located in Hopkinsville, Kentucky.

The community offers a quality senior living experience on our lovely 40-acre tree-lined community with an array of garden home styles, apartments, and services including our 116-bed skilled nursing and rehab area.

Requirements for Admissions Nurse LPN or RN:
  • Licensed Practical Nurse (LPN) in the Commonwealth of Kentucky or Registered Nurse (RN) in the Commonwealth of Kentucky. - Required
  • Associate's degree in nursing or related field from an accredited college or university. - Required
  • 1 year of experience with long-term care admissions; and 1-year performing admissions in acute care, skilled nursing facility, or other healthcare or medical facility. - Required

Responsibilities for Admissions Nurse LPN or RN:
  • Initiates admission assessments on new residents, and hospital returns, and initiates initial plan care.
  • Reviews and coordinates all components of the admission process with the interdisciplinary team.
  • Oversees the auditing records to ensure compliance with facility standards.
  • Participates in discharge planning when necessary.
  • Reviews and evaluates residents' care plans when conditions warrant a change as necessary.
  • Performs required charting according to policies and procedures. Completes Minimum Data Set (MDS), care plans, and other documents as assigned.
  • Maintains the confidentiality of all residents and for sending the file and its information to applicable departments. Ensures file contents include the following, but is not limited to: a) Admission physician orders, History and Physical (H&P), and Progress notes; b) Alternate Physician Form; c) Rehab Potential form; and d) Purified Protein Derivative (PPD) Status Record.
  • Ensures all file contents are approved/signed by the physician; discharge summary and history and physical from the hospital, as applicable; all other forms/documents as required with signatures of resident and/or responsible party.
  • Ensures the new admission file and all the contents are signed by the physician prior to the arrival of the resident. Initiates room readiness checklist and maintains proper follow-up that all items are completed prior to new admissions.
  • Participates in community planning related to the interests of the community, services, and needs of the resident and family.
  • Assist as needed on the unit.
  • Personifies outstanding customer service to co-workers, clients, and vendors by acting as a subject matter resource in a timely, considerate, and friendly manner, always maintaining the "Golden Rule" approach; practices patience and kindness.
