Admission Liaison

Here at Shelby Ridge we exist to enhance the lives of those we serve! We are looking for someone to join our family and develop relationships with our hospitals, senior centers, and our residents.

Job Duties include, but are not limited to:

- Assessing patients at the hospital

- reviewing medical records

- explaining the facilities services provided

- Coordinating care transition from the hospital to the facility

- Verifying Health insurance and/or explaining Medicaid benefits, as well as how to apply for those benefits

- Building a sense of community

-Coordinating fun community events

-Coordinating events for, and with our partners
