The Acute Clinical Lead is accountable for supporting standardized training and orientation for new acute care nurses, as well as support acute care nurses (ACN) that may need additional training and support on executing the fundamentals of our acute and post-acute care coordination standards. This position requires regular local travel to all 5 Philadelphia offices as well as local area hospitals.
The Acute Clinical Lead will work with other members of the Hospital & Community Care Team (HCT), to identify areas of opportunities and improvement to our acute care coordination efforts across the continuum, in support of the ChenMed Mission and Vision.
The Acute Clinical Lead role will also provide coverage for the Acute Clinical Manager, as well as the ACN, Data Quality Coordinator and as needed, for open positions, PTO, etc. This role will serve as a back up to the Acute Clinical Manager role if needed on inpatient census calls, payor census discussions or any other meeting pertinent to our acute or post-acute care coordination efforts. This role will also educate key team members on how to foster relationships with patients' families and care givers, as well primary care physicians, specialists, other care providers, social workers, other case managers and nurses that support our patients in acute and post-acute facilities, home health care companies, and health plans.
This position will also focus on supporting the execution of our teams when conducting onsite hospital visits, communicating and coordinating care with hospitalist/hospital staff, patients and families, as well influencing the appropriate level of care recommendation for our patients. (inpatient vs observation and post-acute care), This position will use our internal charting system to report daily, on inpatient updates and will work with the hospital team on an expeditious discharge planning to next level of care. The Acute Clinical Lead will educate and train ACN to anticipate the need for post-acute and/or long-term care, from day one (1) of hospital stay, providing support to all parties involved. Daily updates in our charting system are required on each patient using the hospitals EMR system and onsite reviews.
The Acute Clinical Lead will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
  • Supports the Acute Clinical Manager on our day-to-day execution of our acute and post-acute care coordination effort as well educating ACN's on these standards.
  • Identifies and validates appropriateness of inpatient vs. observation status
  • Identifies areas of opportunity regarding proper allocation of healthcare resources in an acute and post-acute setting.
  • Identifies and manages safety risk (complete a social assessment), identify functional status (ADLs and PT needs), discuss medications and self-management, identify and correct knowledge deficits.
  • Implements the ACN Coaching program with the appropriate patient population.
  • Identifies from day one (1) of hospital stay any barriers for a safe discharge back to the community
  • Seeks assistance from ChenMed's Specialists when needed to support the care of our patients in healthcare facilities.
  • Coordinates the transition to a lower level of care as soon as appropriate, using a preferred provider if further services are needed.
  • Facilitates discharge to appropriate level of care and preferred providers
  • Communicates discharge with established handoff to all stakeholders including patient, patient's family or designee, PCP, Center Manager and Community Care Nurse.
  • Documents the appropriate date that the patient is medically discharged and update as appropriate.
  • Administers Social Determinates of Health (SDoH) screening with each patient on every admission and communicates to our Community Social Workers or PCPs when a need is identified as well as ensure this is documented is in our internal documenting tool.
  • Identifies new diagnosis during acute stay and provide PCP documentation to review and add to patient problem list.
  • Contacts the center manager or designee to arrange for a 4-day follow-up PCP appointment prior to discharge and whenever possible, communicates this information to the patient/caregiver.
  • As appropriate, offers and discusses with patients' the benefit of our CCM or DM programs and identify patient interest in participation.
  • Coordinates acute UR physician meetings in cities/markets where applicable.
  • Provides supervisory coverage for the Acute Clinical Manager as needed.
  • Provides workday support review appropriate OT, PTO for Acute Care Managers
  • Assists with creation and implementation of SOPs.
  • Participates in surveys, studies and special projects as assigned.
  • Attends meetings as assigned by supervisor
  • Serves as preceptor when assigned
  • Performs other duties as assigned and modified at manager's discretion.
KNOWLEDGE, SKILLS AND ABILITIES:
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Critical thinking skills required.
  • Ability to work autonomously is required.
  • Ability to monitor, assess and record patients' progress and adjust and plan accordingly.
  • Understanding utilization review and how to leverage with inpatient staff for possible reduction of medical cost on long length of stay patients.
  • Ability to plan, implement and evaluate individual patient care plans.
  • Knowledge of nursing and case management theory and practice.
  • Knowledge of patient care charts and patient histories.
  • Knowledge of clinical and social services documentation procedures and standards.
  • Knowledge of community health services and social services support agencies and networks.
  • Organizing and coordinating skills.
  • Ability to communicate technical information to non-technical personnel.
  • Proficient in Microsoft Office Suite products including Excel, Word, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software.
  • Ability and willingness to travel locally, regionally and nationwide up to 10% of the time.
  • Spoken and written fluency in English, bilingual preferred.
EDUCATION AND EXPERIENCE CRITERIA:
  • Associate degree in Nursing required.
  • Register Nurse with bachelor's degree in a related clinical field preferred.
  • A valid, active Registered Nurse (RN) license in State of employment required.
  • Experience managing and leading multidisciplinary teams which may have included RNs, social workers or licensed practice nurses preferred.
  • A minimum of 3 years' clinical work experience required, preferred in a hospital or post-acute care setting.
  • A minimum of 2 year of utilization review and/or hospital-based case management, or hospital discharge planning experience is required and highly desired.
  • Certified Case Manager certification is preferred. Certification through the Commission for Case Manager Certification (CCMC) or the American Association of Managed Care Nurses (CMCN) desired.
