Upland Rehabilitation Care Center is a state-of-the-art skilled nursing and subacute facility featuring 198 beds located in Upland, CA. We have 4 different units of patient care to help best fit your interests and we are constantly looking for great employees to promote! We are looking to fill our Activities Assistant position for our day shift.
Duties:
- Encourage residents to participate in activities.
- Assist in planning, developing, organizing, implementing, supervising, and evaluating the activity programs of this facility.
- Provide materials as necessary.
- Make routine visits to residents and perform assistance with crafts, projects, etc., as necessary.
- Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service.
