Who We Need

As a member of the Assertive Community Treatment (ACT) Team, the Substance Abuse Specialist works collaboratively with a multi-disciplinary care team to identify, assess, and provide stage-wise engagement, persuasion, active treatment, and relapse prevention/management services in individual and group formats for persons with alcohol, tobacco, and other substance use involvement. Works as a collaborating ACT team member to facilitate recovery and inspire hope for service members while supporting their achievement of service plan goals. to assess service member needs, participate in the development of individual service plans, and provide advocacy and supports for service members, their families and involved community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage on an established schedule.

MINIMUM QUALIFICATIONS:

  • High School diploma/GED and four plus (4+) years direct care experience, or
  • Any combination of direct Behavioral Health experience/education totaling five (5) years of experience is acceptable. Example: Bachelor’s degree with one (1) year of experience.
  • Bilingual (Englsh/Spanish) preferred.
  • Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.
  • Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
  • TB Test clearance required at the time of hire and annually. 
  • Must pass a pre-employment physical clearance at the time of hire.

JOB SUMMARY (Including, but not limited to)

  • Supports the Lead Substance Abuse Specialist in implementation of an integrated co-occurring program for adults with SMI consistent with evidence-based program standards. Delivers stage-wise treatment groups, outreach and engagement interventions in clinic and community settings under the oversight of a licensed BHP. Conducts regular reviews of progress and provides status updates to the ACT team. Identifies and implements program strategies for non-responders. 
  • Provides services consistent with PIR policies and productivity standards, including direct counseling and treatment; coordination of care with team members, medical staff and involved community partners and families; develops assessments, crisis plans and service plans; conducts regular review of individual progress and provides recommendations consistent with a stage-wise treatment approach.
  • Provides mobile medication supports to members in their home and other community settings. Observes and documents medication reactions, including adverse reactions and symptom stability, consistent with PIR protocols, and provides regular updates to the ACT Team psychiatrist on progress in care.
  • Assumes “primary contact” responsibility for a smaller portion of service members to assure accurate, thorough, and current documentation in the medical record and consistent therapeutic contact for these members. Maintains a weekly client schedule for this group and serves as first contact for those service members and their families. Provides or arranges transportation on to support primary contact member service needs.
  • Participates in daily ACT Team meetings as well as any additional individual treatment teams or other team communication meetings as required.
  • To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department, consistent with Copa Health productivity levels
