Form of workEndeavor Health Services is seeking a dedicated Access and Triage Specialist to join our dynamic mental health services team. This position will be responsible for developing capacity and workflows to ensure individuals are being screened and assessed for all available services, and are accessing these services in accordance with CCBHC (Certified Community Behavioral Health Clinic) services expectations across sites. This position will collect data, track the process overall and utilize Quality Improvement and Quality Assurance principles to ensure standards are met and the process uses a person centered approach.
Responsibilities
- Utilize a person-centered approach to engage clients and provide education about services available
- Create relationships with community resources for coordination of care, follow up, and linkages
- Provide updates and communication to all members of treatment team
- Be proficient in CCBHC standards of care
- Engage and educate others about services that are available
Education and/or Experience
- Bachelor's degree (or higher) in a related field
- Strong grounding in QI, QA and process
- Proficient and creative with technology
- 1 year experience preferred

Salary starting at $26.00-27.00/hour depending on education, credential and experience.
