Position: RN Case Manager
Location: Various locations on Maui (hybrid)
Pay: $46/hr
Experience: Minimum one (1) year of recent experience in general medical/surgical nursing
Education: Graduate of an accredited school of nursing; Current BLS certification and Registered Nursing License in the State of Hawaii
Type: Full-time; Contract
Schedule: Monday – Friday, Day shift
Kahu Malama Nurses is seeking an RN Case Manager to join a growing and dynamic team!
Job Description:
- Knowledge of the patient’s diagnoses, limitations, plan of health, and current status
- Take phone orders from physicians and transcribe them to a patient's chart, treatment, or health plans
- Conduct physical assessments of the patient, determine their needs, and implement appropriate care
- Complete all paperwork and case management by time-sensitive deadlines
- Collaborate with physicians, supervisors, therapy services, consultants, and agencies to improve the quality of services and to resolve identified problems
- Ensure family and patient concerns are followed up promptly and inform the clinical manager about issues
- Make recommendations for supplies, equipment, etc. to patients and family members
- Perform minor housekeeping duties and maintain safety protocols with other staff members
- Attend meetings, educational programs, and workshops
- Other duties as assigned or requested
Position Requirements:
- Must have reliable transportation
- Knowledge of medical terminology and abbreviation (required)
- Knowledge of state and federal regulations governing home health services (preferred)
- Strong communication and interpersonal skills to enhance positive teamwork and relationships (both internally and externally)
- Ability to use electronic medical records, computer software systems, and applications
- Excellent Microsoft Office suite skills
