Location: either Nassau or Suffolk county NY
Travel to 6 houses per day to check up on patients and do necessary requirements. will be in one specific county not traveling between the two.
Under the supervision of a Registered Nurse, the LPN/LVN(Licensed Practical/Vocational Nurse) – Home Health provides skilled one-on-onenursing care to patients in their home and accompanies them when indicated toappointments, etc.
Essential Duties and Responsibilities:
- Documents each shift or visit to reflect accurately theprovision of skilled nursing care rendered in accordance with physician’sorders.
- Changes in patient condition are identified and documentedwith resolution on the appropriate clinical form. Director/Supervisor notification if applicable
- Physician notification to obtain necessary orders forintervention(s) per state regulations
- Performs specific treatments and medication administrationin accordance with physician orders and Home Health agency’s protocols
- Documentation reflects patient collaboration andparticipation in their plan of care
- Requests for modification of orders and additional ordersare made based on the patient’s clinical status and skilled nursing judgment
- All physician collaboration/coordination of care isdocumented in the medical record and communicated as needed
- Provides patient and family education as appropriate
- Attends case conferences, team meetings and educationin-services as required
Minimum Requirements:
- Graduate of an accredited practical or vocational nursingprogram
- Currently licensed as a Licensed Practical/Vocational Nursein the state in which the LPN/LVN will practice
- One year prior professional nursing experience, home careexperience preferred
- Complies with all relevant professional standards ofpractice
- Participation and completion of Maxim's Competency programwhen applicable
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
- Must meet all federal, state and local requirements
- Successful completion of new hire training as applicable tojob site
- Understand patient confidentiality and HIPAArequirements
- Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
