PRIMARY PURPOSE:
The role of the Medical-Wellness Social Worker involves coordinating and delivering medical and wellness services to MCISD employees of all ages, both at the workplace and in their homes.
QUALIFICATIONS:
Education/Certification:
- Bachelor's degree in social work from a Council on Social Work Education {CSWE) accredited program.
- Master's degree preferred.
- Valid driver's license and personal transportation for conducting home visits.
- Must hold a valid license for Social Worker required
- Proficient in casework methods and case management
- Exceptional communication skills for effective interaction with district staff, community agencies, and other stakeholders
- Familiarity with prevention and intervention strategies, including expertise in behavior management interventions
- Strong organizational, interpersonal, and communication abilities
- Skilled in presenting information to diverse audiences, including students, parents, and district staff, in one-on• one, small group, and large group settings
- Bilingual fluency in both English and Spanish
- A minimum of three years of social work experience in a healthcare setting, preferred
MAJOR RESPONSIBILITIES AND DUTIES:
- Establishes and oversees the social service program, meticulously crafting comprehensive policies, procedures, and guidelines. Maintains an extensive grasp of referral resources spanning the service area.
- Conducts thorough assessments of patients' medical conditions. Collaborates closely with the Director for Payroll, Employee Benefits & Risk Management to relay patient progress, needs, concerns, and potential issues. Ensures precise and timely record-keeping, encompassing clinical and progress notes, in alignment with departmental protocols.
- Collaborates closely with healthcare team members to forge treatment plans, including discharge strategies, anchored in medical evaluations and recognized social work interventions.
- Cultivates collaborative ties with influential figures in community resource organizations, actively representing the agency in meetings and committees addressing patients' social needs
- Provides knowledge to the Employee Benefits team, delivering in-service training and consultation to enhance understanding of social and emotional determinants impacting health conditions.
- Stays abreast of industry developments, encompassing Medicare, Medicaid, and other third-party reimbursement avenues.
- Participates actively in designing and executing quality improvement initiatives.
- Demonstrates exceptional customer relations during interactions with patients, families, physicians, colleagues within the department, other hospital units, referral resources, service agencies, and external parties.
- Champions and facilitates health and well-being initiatives.
- Designs, implements, and supervises comprehensive wellness programs catering to employees' physical, emotional, and mental well-being.
- Tailors wellness strategies to match the unique needs and preferences of district personnel.
- Pioneers initiatives encompassing physical fitness, nutrition, mental well-being, stress management, and other facets of well-being.
- Orchestrates regular health and wellness campaigns, fostering awareness and delivering education on health-oriented lifestyle choices. Coordinates workshops, seminars, and events addressing health-related topics.
- Supervises the seamless execution of wellness programs, collaborating harmoniously with diverse departments, vendors, and external partners.
- Offers resources and tools that bolster individuals on their journey to improved well-being. Coordinates fitness classes, mental health resources, nutritious eating guidelines, and more.
- Assumes a role as a trusted advisor for individuals seeking counsel on health and wellness matters.
- Collaborates closely with the Director for Payroll, Employee Benefits & Risk Management in shaping and updating the strategic blueprint for the Self-Funded Health Insurance program.
- Monitors progress and effectiveness of wellness initiatives through SMART Goal setting, data collection, and astute analysis.
- Conducts surveys, evaluations, and data analysis to pinpoint wellness requirements and preferences among the target audience. Utilizes insights to refine programs and address pertinent health concerns.
- Integrates wellness endeavors seamlessly into the organizational culture and strategic direction, collaborating harmoniously with other departments, management, and stakeholders.
- Coordinates dynamic wellness events, challenges, and competitions, fostering active engagement and motivation among participants on their well-being expedition.
- Adheres rigorously to Mission ClSD policies
- Follow district safety protocols and emergency procedures.
- Performs all duties in a safe manner to avoid injury to oneself and/or others.
- Performs other duties as requested by immediate supervisor.
SUPERVISORY RESPONSIBILITIES:
May supervise persons completing practicum or internship through a college or university program.

Tools/Equipment Used: Personal computer and peripherals; standard instructional equipment
Posture: Prolonged sitting; frequent standing, kneeling/squatting, bending/stooping, pushing/pulling, and twisting
Motion: Frequent walking; repetitive hand motions including frequent keyboarding and use of a mouse; occasional reaching Lifting: Regular light lifting and carrying (under 15 pounds); Occasional heavy lifting (45 pounds and over); may be required to lift and transfer students to and from a wheelchair or assist with positioning of students with disabilities
Environment: Work inside, regular travel to staffs homes.
