NYCHA Expansion Coordinator

PROGRAM OVERVIEW (Public Health Corps)
The NYC Public Health Corps (PHC) is a citywide investment in and commitment to the public health workforce and just recovery from COVID-19 with and for communities that have been disproportionately impacted. Following several years of COVID-19 specific work, the next phase of PHC aims to focus on chronic disease prevention and management through decreasing inequities in health and social outcomes for residents of public housing. Program aims include:
Reduce the inequities in modifiable risks for chronic disease among public housing residents and residents in Bureau of Neighborhood Health catchment areas.
Prevent the unfair impact of public health emergencies including COVID-19 and other respiratory viruses on public housing residents and residents in Bureau of Neighborhood Health catchment areas (Harlem, South Bronx, and Central Brooklyn) by promoting vaccination and other preventive measures.
Screen for social needs and connect residents to services.
Screen for mental health needs and facilitate connection to mental health services.
Demonstrate the impact of community health workers on addressing health inequities to inform Medicaid policy and priorities.
The Health Advocacy Partners Program, a component of PHC, will drive equitable pandemic recovery in priority public housing communities that have experienced longstanding health inequities. The program will fund community-based organizations to build up and utilize a workforce of community health workers (CHWs) to deeply engage with targeted NYCHA neighborhoods in preventing and managing chronic conditions like asthma, hypertension, and diabetes. The program provides:
One-on-one health coaching sessions
Individual and group-level educational workshops
Assistance in acquiring and navigating health insurance.
Referrals to clinical and social support services in the community
The program model builds on more than a decade of work and program data demonstrating impact and positive outcomes in several communities in Harlem.
POSITION OVERVIEW
DOHMH is seeking a NYCHA HAP Expansion Coordinator who will be activated to work at NYCHA's Department of Resident Health Initiatives leading and supporting key aspects of the HAP Expansion program including community and stakeholder engagement to establish CHW interventions at target developments. This position will be funded through June 30, 2024, with possibility of extension. 90 Church Street, 5th Floor, Manhattan, New York 10007. Travel to partner work locations, including DOHMH at 42-09 28th Street, Long Island City, New York 11101.
RESPONSIBILITIES
Work with project partners to engage community stakeholders to advance project planning and continuous quality improvement and success.
Work with project partners, including NYCHA's Department of Resident Economic Empowerment and Sustainability, to recruit CHW staffing for the program.
Serve as a liaison to advance project coordination with other NYCHA departments NYCHA teams, community leaders, and partners.
Work with NYCHA internal and external partners to ensure the work of DOHMH staff, CBO partners, and other project partners are optimally aligned with related special initiatives across the target developments.
Coordinate access to relevant training and related tools for project staff on housing topics.
Facilitate the activation of NYCHA resident and partner communications tools to advance program deliverables.
Work with project partners to optimize integration and synergy with other NYCHA place-based community health worker (CHW) initiatives.
Collaborate with DOHMH and others project stakeholders to increase the visibility of place based CHW investments that promote health equity, improve health outcomes, and identify and help address barriers to accessing quality care.
Work with NYCHA, DOHMH and other colleagues to conceptualize a continuing education learning and networking community for NYCHA resident community health workers to support their continued advancement in the preventive health field.
Lead coordination to integrate HAP expansion with other NYCHA Resident Health Initiatives related to urban agriculture & food security, maternal and birth equity, smoke-free housing, and mental health.
QUALIFICATIONS
Experience managing relationships with partners that work in and serve disinvested neighborhoods.
Experience applying a health equity lens to public health programming required.
Knowledge of capacity building strategies with a racial justice and health equity lens
Experience with workforce development, leadership development, or professional development/training
Knowledge and experience working with diverse community-based organizations and public agencies.
Leadership skills in coalition building
Creative, flexible, able to meet tight deadlines, and able to manage several tasks at once.
Good problem-solving skills
Excellent interpersonal and communication skills
Current or former public housing residents or others with experience in public housing communities strongly encouraged to apply.
SALARY
Salary range is $70,000 to $ 75,000.
