The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. Community Engagement Manager, Health Advocacy Partner

PROGRAM OVERVIEW (Public Health Corps)

The NYC Public Health Corps (PHC) is a citywide investment in and commitment to the public health workforce and just recovery from COVID-19 with and for communities that have been disproportionately impacted. Following several years of COVID-19 specific work, the next phase of PHC aims to focus on chronic disease prevention and management through decreasing inequities in health and social outcomes for residents of public housing. Program aims include:

  • Reduce the inequities in modifiable risks for chronic disease among public housing residents and residents in Bureau of Neighborhood Health catchment areas.
  • Prevent the unfair impact of public health emergencies including COVID-19 and other respiratory viruses on public housing residents and residents in Bureau of Neighborhood Health catchment areas (Harlem, South Bronx, and Central Brooklyn) by promoting vaccination and other preventive measures.
  • Screen for social needs and connect residents to services.
  • Screen for mental health needs and facilitate connection to mental health services.
  • Demonstrate the impact of community health workers on addressing health inequities to inform Medicaid policy and priorities. 

The Health Advocacy Partners Program, a component of PHC, will drive equitable pandemic recovery in priority public housing communities that have experienced longstanding health inequities. The program will fund community-based organizations to build up and utilize a workforce of community health workers (CHWs) to deeply engage with targeted NYCHA neighborhoods in preventing and managing chronic conditions like asthma, hypertension, and diabetes.  The program provides:

  • One-on-one health coaching sessions
  • Individual and group-level educational workshops
  • Assistance in acquiring and navigating health insurance.
  • Referrals to clinical and social support services in the community

 

The program model builds on more than a decade of work and program data demonstrating impact and positive outcomes in several communities in Harlem.

 

POSITION OVERVIEW

The Community Engagement Manager for the Health Advocacy Partners Program (HAPP) will oversee development and implementation of the Health Advocacy Program in the Bronx, Brooklyn, and Queens public housing developments.  They will be responsible for working closely with community partners in select NYCHA developments and ensuring grant deliverables and targets are met, tracked, and reported. They will be responsible for guiding, supporting, and monitoring contracted community-based partners' delivery of health coaching services, social support referrals, and wellness activities geared towards meeting program goals.  The candidate should be able to demonstrate experience in developing templates, peer-learning opportunities, and liaising between various stakeholders to ensure the success of Community Engagement strategies. Candidates with prior experience and familiarity in working with NYCHA residents will be given greater consideration.

The Community Engagement Manager will report to the Senior Program Manager of HAPP. 

Given the nature of the ever-evolving landscape, some evening and weekend work may be required but compensated. Occasional travel to partners in Brooklyn, the Bronx, and Queens may be needed. Located at 4209 28th St, Long Island City, NY 11101

 

RESPONSIBILITIES

  •  Oversee management of contract deliverables for three community- and faith-based organizations working on NYCHA properties in the Bronx, Brooklyn, and Queens.
  • Manage partner engagement and capacity-building strategies and ensure alignment with overall program goals.
  • Develop templates and guidance for HAPP partners and offer ongoing support in following the guidance.
  • Assist the Senior Program Manager in developing and conducting presentations and training for HAPP partners.
  • Develop and strengthen relationships with HAPP partners and their stakeholders by participating in community events when possible and sharing DOHMH resources.
  • Schedule regular communications with community partners to guide in completing deliverables.
  • Track and process all grant contracts, invoices, and other administrative components to ensure compliance.
  • Assist in the development of program reports to internal and external stakeholders.
  • Support partner reporting needs, data collection, and communications to stakeholders.
  • Provide technical assistance on contracting, invoicing, and other administrative processes to partner organizations.
  • Support partners in navigating NYCHA resources, protocols, and policies.
  • Other duties as assigned by the team. 

 

QUALIFICATIONS

  •  Bachelor's degree in public health, Social Work, Social Services, Public Administration, Human Services, or a related field is preferred OR comparable Community Engagement experience.
  • Strong relationship builder with excellent interpersonal and communication skills
  • Ability to work independently, be flexible, and handle changing workloads with multiple projects and changing priorities.
  • Experience working with highly marginalized populations.
  • A commitment to collective action and community organizing. 
  • Experience planning and implementing neighborhood-based programs.
  • Experience in healthcare and/or social service administration with a content focus on transformation, community/clinical linkages, integration, and/or population health
  • Knowledge of the NYCHA community and public housing landscape
  • Understanding of community health workers and the role they play in health inequities.
  • Ability to communicate effectively across management levels and manage several priorities.
  • Strong oral and written communication skills, especially presentations at and facilitation of community events/meetings
  • Have a flexible schedule that can accommodate travel and participation in some weekday evening and weekend community events.

 

SALARY 

  • Salary range is $85,000 to $90,000.
